Description
Humboldt Senior Resource Center is looking for a full-time Day Center Supervisor to oversee the Day Center for our Redwood Coast PACE program. The schedule for this position is Monday through Friday 8 am to 4:30 pm.
JOB SUMMARY: The Day Center Supervisor is responsible for overseeing the daily operations staff and activities of the Redwood Coast PACE Day Center to ensure a safe engaging and supportive environment for participants. Reporting to the Center Manager this position provides direct supervision and coaching to CarePartners and the Kitchen Aide facilitates training and competency development and ensures the delivery of person-centered care and activities tailored to participant needs. The Supervisor coordinates with the Interdisciplinary Team (IDT) volunteers and community partners to develop and implement meaningful programs while monitoring participant care plans compliance with regulatory requirements and overall program quality. Through leadership program planning and staff mentorship the Day Center Supervisor plays a key role in promoting participant independence wellness and quality of life.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Supervision and Leadership
- Facilitates and supports an effective team approach in implementing PACE guiding principles. Works with management to foster a positive team environment.
- Delegates mentors and supports the CarePartners and Kitchen Aide in activities PT/OT maintenance and meal services.
- Directly supervises CarePartners and Kitchen Aide at the assigned program facility.
- Guides competency evaluations program-orientation ongoing training and performance coaching for employees supervised. Completes the annual evaluations and bi-annual goal reviews for employees supervised.
- Oversees volunteer recruitment orientation training and recognition.
- Acts as a leader and mentor in implementation of Person-Centered Care.
- Reports to the Center Manager concerning operations of the Day Center including situations or occurrences that impact safety and efficiency.
- Facilitates and supports an effective team approach in implementing the programs guiding principles. Attends and participates in the Interdisciplinary Team (IDT) management meetings and other team meetings as requested. Facilitates daily huddles with program employees.
- Supports a person-centered approach to management staffing and program implementation.
- Oversees overall day center activities and services.
- Trains CarePartners in individual and group crafts games and discussion groups as well as planning field trips.
- With the support of the Physical and Occupational Therapist train CarePartners on how to properly implement the therapy plans created for individuals and groups at the Center.
- Facilitates training for CarePartners in the provision of personal care services (Activities of Daily Living).
- Participates as an active member and leader of various department and team meetings including CarePartner team meetings and Interdisciplinary Team (IDT) meetings.
- Attends in-service trainings and presents training topics to program team members and volunteers as requested by management.
Participant Care
- Works with elderly and disabled adults to support their participation in a variety of activities at the Day Center.
- Evaluates the needs abilities and interests of each participant and ensures the development of an individualized plan of care that reflects the principles of Person-Centered Care.
- Collects and considers input from team members and participants about the monthly calendar of activities. Plans for implements posts and distributes monthly calendar of activities.
- Facilitates quarterly participant advisory meetings and reviews individual plans of care to monitor the quality of services.
- Complies with all policy and procedures of Redwood Coast PACE.
Program Development
- Evaluates on a regular basis through participant interviews and team operations meetings participants changing needs. Facilitates implementation of necessary activity program adjustments.
- Researches and implements activities appropriate for specific participant populations. Ensures provision of activities specified in the participants individualized plans of care.
- Completes necessary paperwork and documentation to ensure compliance with applicable regulations.
- Works with Center Manager to purchases activity and day center supplies.
- Maintains storage of supplies and supply areas.
- Assists as needed in the set-up and cleanup of activity supplies. Moves tables chairs and activity equipment for different activities.
Community and Intra-Agency Interactions
- Networks and coordinates with community agencies as appropriate.
- Assists with fundraising activities on behalf of the program and agency including participation in special events and fundraising for specific program purposes.
- Attends agency committees as requested by the Department Director.
- Networks and coordinates with community agencies as appropriate.
- Demonstrates behaviors consistent with agency values: Respect Patient Courteous Tolerant Friendly and Teamwork.
- Additional duties may be assigned as needed that align with the responsibilities of the position and support the vision mission and values of the organization.
Qualifications
EDUCATION &/or EXPERIENCE:
- High school diploma or GED equivalent required.
- The Day Center Supervisor will have one of the following:
- Two (2) years of experience in a social recreational or educational program within the past five (5) years one (1) year of which was full-time in an activities program in a health care mental health or handicapped program setting;
or
- Be an art therapist music therapist dance therapist recreation therapist occupational therapy assistant occupational therapist or a qualified social worker;
or
- Have completed at least 36 hours of training in a course designed specifically for the activity coordinator role and approved by the State department of health services.
. 3. One (1) year of experience working with a frail or elderly population providing personal care in a health facility or in-home setting preferred.
LICENSES OR CERTIFICATES:
- Current CPR and First Aid certification within six (6) months of hire.
- One of the following Certified designations preferred: Therapeutic Recreation Specialist Physical Therapy Aide Nursing Assistant Home Health Aide.
PRE-SCREENING REQUIREMENTS:
- This position requires the successful completion of a background check.
- Pre-employment physical and PPD skin test (TB screening).
- COVID-19 and Influenza (flu) vaccines preferred.

PHYSICAL REQUIREMENTS:
The Therapeutic Services Coordinator role requires frequent standing walking bending and reaching while leading activities and assisting participants. The position involves occasional lifting pushing or moving of supplies activity equipment and furniture such as tables and chairs typically up to 25 pounds. The Coordinator must be able to assist frail or mobility-limited participants safely which may include supporting transfers or guiding them during activities. The work environment is active and requires sustained energy flexibility and the ability to adapt to changing participant needs throughout the day.
