Make a Meaningful Difference-One Visit at a Time Are you a skilled and caring Licensed Practical Nurse (LPN) looking for a stable weekday nursing role with purpose SummitWest Care a well-respected non-profit home health agency in Grand Junction CO is seeking a Full-Time Weekday LPN to join our mission-driven care this role youll bring essential care directly to patients in their homes helping them live independently with dignity and comfort. If youre passionate about building real connections and delivering patient-first nursing care we want to hear from you. Your Responsibilities Deliver personalized in-home nursing care to a diverse patient population Provide services such as wound care medication administration catheter care and more Monitor and report changes in patient condition to the RN and care team Support chronic disease management and educate patients and families Maintain accurate timely documentation Travel locally within Mesa County (mileage reimbursed) What Youll Need Active LPN license in the State of Colorado IV Certification a plus! Home health experience Strong clinical skills organization and communication Reliable transportation valid drivers license and auto insurance A compassionate heart and a passion for helping others thrive at home
