Your Responsibilities Deliver personalized in-home nursing care to a diverse patient population Provide services such as wound care medication administration catheter care and more Monitor and report changes in patient condition to the RN and care team Support chronic disease management and educate patients and families Maintain accurate timely documentation Travel locally within Mesa County (mileage reimbursed) What Youll Need Active LPN license in the State of Colorado IV Certification a plus! Home health experience Strong clinical skills organization and communication Reliable transportation valid drivers license and auto insurance A compassionate heart and a passion for helping others thrive at home
