UMass Memorial Medical Center is seeking an experienced Nurse Practitioner or Physician Assistant to join our growing Advanced Therapeutics Cardiometabolic (CMB) and Medical Weight Loss (MWL) programs. As part of an academic health system this remote role offers the opportunity to deliver cutting-edge patient-centered care to a complex and expanding patient population.
With a surge in referrals and the rising complexity of care needs our programs are expanding to meet demand. This Advanced Practice Provider (APP) will play a pivotal role in managing patients requiring intensive medication management close safety monitoring and coordination across multidisciplinary teams.
Major Responsibilities:
Provide comprehensive telehealth visitsfor patients with obesity diabetes and cardiometabolic conditions.
Initiate and manage advanced therapeutics(GLP-1s dual incretin therapies and other weight management agents).
Monitor labs adjust medications and ensure safe effective treatment.
Educate and empower patients on lifestyle disease management and medication use.
Collaborate with a multidisciplinary care team(physicians pharmacists dietitians behavioral health).
Support patients with medication access prior authorizations and ongoing adherence.
Contribute to program growth and quality improvement initiatives.
Qualifications:
Graduate of approved Nurse Practitioner or Physician Assistant program with certification (Masters Degree).
Massachusetts NP/PA licensure.
BLS and ACLS certification required.
Provider experience in cardiology or medical weight loss (GLP1)
Strong communication organizational and critical thinking skills.
Experience withEpic EMRis a plus.
Must maintain a private professional workspace with reliable high-speed internet and video capabilities.

Schedule:
32-hours per week
4 (8-hour) day shifts
100% remote
