Nurse Practitioner / Physician Assistant

Valley Health is seeking a compassionate and dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our new Center for Senior Wellness team. This role is designed to support our mission of delivering high-quality patient-centered care for seniors right where they live.
About the Role
The NP/PA will provide comprehensive healthcare services with a strong emphasis on home visits for seniors. This position offers the opportunity to build trusted relationships with patients and families ensuring care is accessible personal and coordinated.
Key Responsibilities
- Conduct home visits to assess diagnose and manage patients health conditions
- Develop implement and evaluate care plans in collaboration with the care team
- Provide preventive care chronic disease management and patient education
- Collaborate with physicians nurses and other team members to ensure continuity of care
- Document visits and maintain compliance with regulatory standards
Qualifications
- Current licensure as a Nurse Practitioner or Physician Assistant in the state of West Virginia
- Certification and prescriptive authority required
- Experience in primary care geriatrics or home health preferred
- Strong interpersonal and communication skills with a patient-first mindset
- Ability to work independently in the field while staying connected to the care team
