Description

JOB DESCRIPTION: The Medication Safety Director is responsible for providing leadership in overseeing the safe use of medications within the organization. The Medication Safety Director will utilize effective communication and interpersonal skills to collaborate with pharmacy nursing medical staff IT and hospital administration leaders to improve the medication use process through fostering a culture of safety participating in retrospective and proactive risk identification and facilitating process improvement/quality improvement initiatives to reduce opportunities for error. Collaborate with entity and system-level pharmacy leadership to implement best practices for medication safety. Monitor evidence-based trends and best practices guidelines and positions for medication safety and work collaboratively to facilitate implementation of recommendations. Assist entity pharmacy leadership with monitoring compliance in adherence to medication safety-related policies and Joint Commission Medication Management standards. Establish and track medication safety related metrics for presentation to various stakeholders in the system. Coordinates system-level monthly Medication Safety Committee meetings. Identifies initiatives that require escalation to system level governing bodies for approval when necessary. Prepares quarterly reports of medication safety initiatives and activities for entity and system level stakeholders. Ensure appropriate follow-up and implementation of initiatives as identified and approved. Reviews all medication-related patient safety events. Investigates reported errors where appropriate and facilitates intra- and interdepartmental follow-up in coordination with entity-level pharmacy teams. Collaborates with multiples patient safety-oriented committees to review medication errors based on investigation and follow-up of reported errors. Collaborates with Office of Quality and Risk Management to review investigate and implement system changes when appropriate for errors based on event severity and likeliness for reoccurrence. Works collaboratively to update and maintain infusion pump drug libraries. Establishes program for ensuring continuous quality improvement for infusion safety including data monitoring with recommendations. Performs other duties as assigned or requested.

MINIMUM QUALIFICATIONS: Graduate of an ACPE-accredited school of Pharmacy with B.S. Pharmacy or PharmD degree. PGY2 in Medication Use Safety Health-System Pharmacy Administration or Drug Information (with emphasis on ownership for medication safety activities and implementation of large-scale medication safety initiatives) or equivalent experience. Five years of extensive experience in medication safety or administration (with a focus in medication safety). Minimum of 3 years experience in a leadership position. Experience within a large academic Healthcare System preferred. Current Georgia Pharmacist License or eligibility for Georgia licensure.

PHYSICAL REQUIREMENTS (Medium): 20-50 lbs; 0-33% of the work day (occasionally); 11-25 lbs 34-66% of the workday (frequently); 01-10 lbs 67-100% of the workday (constantly); Lifting 50 lbs max; Carrying of objects up to 25 lbs; Occasional to frequent standing & walking Occasional sitting Close eye work (computers typing reading writing) Physical demands may vary depending on assigned work area and work tasks.

ENVIRONMENTAL FACTORS: Factors affecting environment conditions may vary depending on the assigned work area and tasks. Environmental exposures include but are not limited to: Blood-borne pathogen exposure Bio-hazardous waste Chemicals/gases/fumes/vapors Communicable diseases Electrical shock Floor Surfaces Hot/Cold Temperatures Indoor/Outdoor conditions Latex Lighting Patient care/handling injuries Radiation Shift work Travel may be required. Use of personal protective equipment including respirators environmental conditions may vary depending on assigned work area and work tasks
