Job Summary:
This Physical Therapist position involves providing outpatient inpatient and skilled nursing facility (SNF) care in a critical access hospital setting. The role requires a minimum of one year of experience and includes responsibilities such as patient assessment treatment planning and implementation of therapeutic interventions. The facility provides housing and a vehicle for the duration of the assignment.

Responsibilities:
- Provide physical therapy services to patients in outpatient inpatient and SNF settings.
- Develop and implement individualized treatment plans.
- Assess patient needs and progress.
- Document patient care and progress notes.
- Collaborate with other healthcare professionals.
- Maintain accurate records and billing information.
- Adhere to all facility policies and procedures.
Required Skills & Certifications:
- Current Physical Therapist license.
- Minimum of one year of experience as a Physical Therapist.
- BLS certification.
- Experience in outpatient acute care and LTC/SNF settings.

Special Considerations:
- No on-call requirements.
- No floating between units.
- Block scheduling may be accommodated.
- Facility provides housing (1-bedroom apartment no pets allowed) and a vehicle (shared-use within 30-mile radius).
- Housing and vehicle available 72 hours before and until 24 hours after contract ends.
- Traveler must provide a valid drivers license and proof of insurance.
- Personal vehicle insurance must include rental car coverage.
- Time off approved individually.
- Guaranteed 70 hours per two weeks; one 10-hour shift may be cancelled per two weeks.
- Candidates must live at least 50 miles from the facility.
- Must be separated from any Providence facility employment for at least 3 months prior to this assignment.
- Pending licenses are accepted.
Scheduling:
- 5 eight-hour days per week.
- 70 guaranteed hours per two weeks.
