Job: Licensed Master Social Worker Classification: Hourly/Non-Exempt Job Category: 2 Professionals Position Summary The Inpatient Licensed Master Social Worker (LMSW) is responsible for providing comprehensive psychosocial assessments crisis intervention and discharge planning for patients in an acute hospital setting. The LMSW collaborates with the healthcare team to address the social emotional and financial needs of patients and their families. This role is essential to ensuring safe transitions of care especially in rural communities where resources may be limited and access to care is a significant concern. Essential Functions -Conduct psychosocial assessments for patients and families to determine needs related to discharge planning and continuity of care. -Develop and implement individualized discharge plans that address medical emotional financial and social needs. -Provide crisis intervention supportive counseling and referrals to community resources. -Coordinate with medical staff nursing and case management to ensure patient-centered care. -Serve as a liaison between the hospital patients families and external agencies. -Document all social work activities in the electronic health record (EHR) accurately and timely. -Advocate for patient needs related to housing transportation insurance and access to outpatient services. -Assist in cases of abuse neglect domestic violence and end-of-life care planning. -Facilitate family meetings and patient education sessions as needed. -Participate in quality improvement initiatives related to care transitions and social determinants of health. -Attends regularly scheduled utilization review/discharge planning meetings. -Demonstrates effective oral and written communication skills. -Demonstrates counseling skills. -Adheres to and follows all patient experience initiatives. -Reports to and departs from work on time and as scheduled. -Performs other duties as assigned. -Must be able to deal with stress while maintaining composure. -Follows and adheres to TRMC vaccine policy(s) mandated by the Centers for Medicare & Medicaid Services (CMS). Skills & Competencies -Strong assessment problem-solving and crisis intervention skills. -Ability to navigate complex psychosocial situations with compassion and professionalism. -Familiarity with community resources Medicaid/Medicare and discharge planning protocols. -Excellent communication documentation and organizational skills. -Cultural sensitivity and understanding of the unique challenges faced by rural populations. Work Experience -One to two years of social work experience in a hospital setting preferred. -Experience in rural or underserved settings strongly preferred. Education -LMSW Licensed by the Texas State Board of Social Workers required. -BLS (Basic Life Support) within 30 days of hire date.
