Burnett Medical Center is currently looking for a Social Services Assistant to help in our busy Social Services department.
This position includes benefits (medical dental vision employer-paid life insurance short-term disability along with up to a 3% match in our 401k retirement fund.
SHIFT & HOURS: Full-time Monday thru Friday 8am- 4:30pm
RESPONSIBILITIES:
Participate in development and ongoing review and coordination of services for residents and patients providing assistance in areas of patient/resident care discharge planning and home planning processes with other facility departments external organizations agencies and healthcare facilities. Other duties as assigned.
Requirements
QUALIFICATIONS
- Associate or Bachelors degree preferably in human services social work and/or nursing.
- Experience in social services long-term care or healthcare.
- Knowledge of community and extended community resources and able to network with those providers.
- Team player positive attitude strong people skills verbal and written communication skills ability to adapt to change in a fast-paced environment.
- Extensive experience with technology including email calendars word processing and office machines.
- Experience with use of EHR software programs.
