Description

Burnett Medical Center is currently looking for a Social Services Assistant to help in our busy Social Services department.

This position includes benefits (medical dental vision employer-paid life insurance short-term disability along with up to a 3% match in our 401k retirement fund.

SHIFT & HOURS: Full-time Monday thru Friday 8am- 4:30pm

RESPONSIBILITIES:

Participate in development and ongoing review and coordination of services for residents and patients providing assistance in areas of patient/resident care discharge planning and home planning processes with other facility departments external organizations agencies and healthcare facilities. Other duties as assigned.

Requirements

QUALIFICATIONS

Associate or Bachelors degree preferably in human services social work and/or nursing.
Experience in social services long-term care or healthcare.
Knowledge of community and extended community resources and able to network with those providers.
Team player positive attitude strong people skills verbal and written communication skills ability to adapt to change in a fast-paced environment.
Extensive experience with technology including email calendars word processing and office machines.
Experience with use of EHR software programs.
