Summary

Social Worker - Road to Recovery Program at Providence Health Care

Are you a Social Worker passionate about making a real impact on individuals facing substance use and mental health challenges Providence Health Care is hiring for the Road to Recovery Program in a Regular Part-Time position.

Join our innovative interdisciplinary team providing holistic person-centred trauma-informed care. With the new stabilization unit opened in Fall 2023 and continued program expansion youll be part of a cutting-edge approach to recovery. Our program offers detox beds transitional care aftercare outreach services ensuring clients receive the right support at every stage of their journey.

In alignment with the Mission Vision Values and strategic directions of Providence Health Care the person in this role will promote a safe respectful and civil working environment for patients residents families visitors and staff.

Reporting to the designated Social Work Site Leader and receiving guidance and support from the Professional Practice Leader and/or Research Education and Practice Coordinator the position supports the interdisciplinary program/team which is under the direction of the Patient Care Manager in achieving program patient and resident goals.

In accordance with the Mission Vision and Values and strategic directions of Providence Health Care the person promotes a safe respectful and civil working environment for patients residents families visitors and staff.

  • Promote a safe respectful and civil working environment for patients residents families visitors and staff in alignment with the Mission Vision Values and strategic directions of Providence Health Care.
  • Report to the Professional Practice Leader and work under the supervision of the designated Site Leader.
  • Collaborate with other Site/Professional Practice Leaders for the interdisciplinary program team.
  • Provide advanced therapeutic clinical counseling education and resource services to individuals and families affected by problematic substance use.
  • Formulate and implement treatment plans providing comprehensive case consultation and follow-up support on treatment decisions in collaboration with the interprofessional team.
  • Provide leadership in addressing complex care needs during the discharge planning process and determine effective utilization of resources.
  • Participate in the development of community resources to support recovery and well-being.
  • Liaise and consult with the PHC Addiction and Mental Health teams as appropriate.
  • Participate in evaluating treatment practices and protocols within the social work team to promote excellence in social work practice.
  • Develop research protocols and conduct interdisciplinary research and special projects in collaboration with appropriate personnel.
  • Participate as a member of local provincial/federal committees linking hospital programs to community support services.

What You Bring

Qualifications

  • Masters Degree in Social Work from an accredited university.
  • Post-graduate continuing education in therapeutic intervention models related to addictions and mental health.
  • Three (3) years of recent related experience working in the areas of addiction mental health HIV/AIDS and complex pain or an equivalent combination of education training and experience.
  • Current full registration with the British Columbia College of Social Workers (BCCSW)

Key Skills & Knowledge:

  • Strong knowledge of addiction issues and evidence-based treatment practices.
  • Expertise in psychiatric diagnoses including complex concurrent disorders.
  • Effective communication and collaboration with clients families and teams.
  • Understanding of the social determinants of health.
  • Skilled in conducting psychosocial assessments with standardized tools.
  • Ability to plan implement and evaluate interventions based on assessments.
  • In-depth knowledge of community resources and ability to make effective referrals.
  • Experience in case management counseling and therapy for substance use issues.
  • Ability to work independently and as part of an inter-professional team.
  • Experience in working within a culturally diverse environment.
  • Strong organizational and prioritization skills in a demanding setting.
  • Knowledge of relevant legislation including the Mental Health Act and Adult Guardianship Act.
  • Ability to educate and consult through presentations in-services and support groups.
  • Proficient in word processing and database management software.
  • Physical ability to perform job duties.

Key Responsibilities:

  • Provides advanced therapeutic counseling to clients families and groups facilitating change in coping styles behaviors and attitudes while coordinating services and making referrals.
  • Conducts comprehensive psychosocial assessments gathering relevant social data to impact clients coping abilities.
  • Assesses client needs develops care options with client input and facilitates access to appropriate community services and programs.
  • Develops and implements care and treatment plans in collaboration with interprofessional teams ensuring respect for client dignity and independence.
  • Maintains client records including assessments progress notes and reports in compliance with standards and regulatory requirements.
  • Provides referrals to community treatment agencies and services including addiction treatment and rehabilitation programs.
  • Offers clinical consultation to social workers interdisciplinary teams and external agencies.
  • Contributes to the development and evaluation of drug and alcohol programs and makes recommendations to leadership.
  • Coordinates clinical teaching and fieldwork placements for social work students and provides post-academic training.
  • Sets learning goals for professional development in areas such as addiction mental health and complex pain.
  • Engages in practice evaluation identifies improvement opportunities and participates in team or committee activities.
  • Develops and delivers educational sessions presentations and training for clients families and staff.
  • Maintains workload records analyzes trends and recommends action plans to the Professional Practice Leader.
  • Develops research protocols and conducts interdisciplinary research to inform clinical practices.
  • Performs other related duties as required
