osition Overview
The Clinical Data Abstractor will be responsible for accurately performing data abstraction duties across Core Measures including Sepsis Perinatal Care Outpatient Measures and Psychiatric Measures. This role requires strong attention to detail excellent organizational skills and the ability to manage multiple accounts effectively while ensuring compliance with established standards and guidelines.
Key Responsibilities
Review and extract clinical data from patient medical records focusing on disease-specific care such as diagnoses treatments procedures and outcomes.
Validate and ensure the completeness and accuracy of abstracted data by cross-referencing multiple sources within the patient record.
Follow established guidelines and abstraction protocols including those from the Centers for Medicare & Medicaid Services (CMS) and The Joint Commission (TJC).
Enter and maintain clinical data in designated databases or registries using specialized abstraction software.
Participate in quality assurance initiatives including audits to ensure data reliability and integrity.
Stay up to date with evolving standards in clinical care healthcare quality reporting and data abstraction best practices.
Qualifications
Minimum 2 years of recent experience abstracting Core Measures required.
Solid understanding of medical records healthcare documentation billing practices and quality reporting standards.
Ability to work independently while prioritizing and managing multiple assignments.
Strong accuracy detail orientation and analytical skills.
Excellent verbal and written communication skills with the ability to collaborate across teams.
Education: BSN RN or LPN preferred.
Certification: CPHQ preferred.
Must maintain confidentiality and demonstrate professional judgment in all aspects of work.
