You will be updated with latest job alerts via emailThe Agency of Human Services (AHS) Vermont Chronic Care Initiative (VCCI) Outreach and Support Coordinator (OSC) screens Vermonters new to Medicaid to complete a health-related social needs screening providing brief intervention (including a 3 way phone call to establish new patient appointments at health clinic to benefit programs such as food resources and tobacco cessation programs) based on the individuals identified needs and readiness for help with these connections. Based on risk level the OSC may make a direct referral for community-based case management.
The outreach screening completion and brief intervention are completed primarily by phone and may also require follow-up with individuals who may complete the screening by mail or through the patient portal. The candidate must be able to maintain a high level of flexibility rapid response and coordination of details to act as a resource to VCCI. This position engages members in dialogue of health and health related needs utilization of resources and services and supporting members in the navigation of health services.
Other duties based on the needs of the business may be assigned.
The candidate must be able to work independently and work as part of a team be highly skilled in customer service must be able to adapt their communication skills and approaches in response to each individual person and their needs. The ability to use de-escalation techniques and motivational interviewing skills is of high value. Adherence to program workflow and documentation standards is required. Knowledge and skill with Microsoft suites excel is preferred. This position may involve working out in the community and in various offices occasionally.

AHS BACKGROUND CHECKS: Candidates must pass any level of background investigation applicable to the accordance with AHS Policy 4.02 Hiring Standards Vermont and/or national criminal record checks as well as DMV and adult and child abuse registry checks as appropriate to the position under recruitment will be conducted on candidates with the exception of those who are current classified state employees seeking transfer promotion or demotion into an AHS classified position or are persons exercising re-employment (RIF) rights.
Duties primarily performed in office settings and with extensive telephonic contact but may involve some travel to other regional offices for which a private means of transportation is required. State policy dictates that use of the most cost-effective method of travel for the state is used for which access to state fleet pool or use of rental car must be utilized. Incumbents must be able to handle with tact and understanding individuals in a variety of stressful and emotional situations.
Associate degree in Social Work or Human Services related work
Bachelors degree in Social Work with experience in a health care setting /or knowledge of the Vermont Agency of Human Services.
Experience with community collaboration systems of care and team work is strongly preferred and relevant contact with people facing hardship lack of socio-economic resources and those with mental health/substance abuse issues.
