Job Description

Overview

The Agency of Human Services (AHS) Vermont Chronic Care Initiative (VCCI) Outreach and Support Coordinator (OSC) screens Vermonters new to Medicaid to complete a health-related social needs screening providing brief intervention (including a 3 way phone call to establish new patient appointments at health clinic to benefit programs such as food resources and tobacco cessation programs) based on the individuals identified needs and readiness for help with these connections. Based on risk level the OSC may make a direct referral for community-based case management.


The outreach screening completion and brief intervention are completed primarily by phone and may also require follow-up with individuals who may complete the screening by mail or through the patient portal. The candidate must be able to maintain a high level of flexibility rapid response and coordination of details to act as a resource to VCCI. This position engages members in dialogue of health and health related needs utilization of resources and services and supporting members in the navigation of health services.

Other duties based on the needs of the business may be assigned.


The candidate must be able to work independently and work as part of a team be highly skilled in customer service must be able to adapt their communication skills and approaches in response to each individual person and their needs. The ability to use de-escalation techniques and motivational interviewing skills is of high value. Adherence to program workflow and documentation standards is required. Knowledge and skill with Microsoft suites excel is preferred. This position may involve working out in the community and in various offices occasionally.
