Are you an experienced Nurse Practitioner seeking a rewarding career that cares for others in a professional practice that cares for you Youre looking in the right place.

Reporting to the Senior Manager the Nurse Practitioner (NP) position is responsible to provide direct care to patients within the NP legislated scope of practice for patients living within a designated geographical area suffering from palliative complex comorbidities and geriatric needs.

What will you do

  • Manages the delivery of coordinated high quality and clinical expertise to the high intense wait at home program palliative care programs designated Long Term Care Homes (LTCH) and retirement homes (RH) and be responsive to other ED avoidance needs in the community within a designated geographical area
  • Work within the full scope of practice and will autonomously assess diagnose provide pain and symptom management order and interpret diagnostic tests and prescribe pharmaceuticals within the NP legislated scope of practice to improve quality or life for patients
  • Develops initial clinical assessments which include assessing home environment supports available in the home level of health goals and expectations related to care. Assessment may include applications for hospice
  • Develops collaborative relationships with Ontario Health atHome Care Coordinators physicians coroners hospital staff hospice staff LTCH staff Retirement Home staff and other community partners as identified i.e. community support service agencies OHT Community Paramedicine program Paramedic Services Palliative Treat and Refer program etc.
  • Improves the quality of life of individuals and their families by giving them the choice to receive care in their preferred location
  • Manages complex and difficult patient issues that cannot be handled in a routine manner
  • Collaborative consultation with physicians NPs and broader system care team to maintain patient centred care
  • Establishes and maintains effective relationships with designated service providers to deliver quality palliative complex and geriatric care
  • Researches palliative geriatric and complex care needs to provide exceptional care to patients
  • Participates in the teaching and coaching of family members LTCHs RHs to support them with palliative complex and geriatric care
  • Participates and provides effective advice and guidance to Care Coordinators to help manage difficult and unusual cases
  • Documents care interventions consistent with Ontario Health atHome documentation platforms and standards as well as CNO documentation standards
  • Supports data collection and program review consistent with NP objectives
  • Works collaboratively with the broader heath system care team by participating in clinical rounds and care conferences
  • May be required to complete nasopharyngeal swabs for patients in the community or LTCH as required.
  • May be required to complete assessments and or provisions for patients requiring Medical Assistance in Dying as required.
  • Will be required to complete clinical assessments in patient homes LTCHs Retirement Homes etc.
  • Designs and implements materials and strategies for enhancement of clinical competency and capacity of LTCH staff to address clinical complexity in the LTCH environment as applicable.
  • Mentor and assist in orientating new NPs and support other NP programs as directed by leadership

What must you have

  • Masters degree any relevant field
  • Current RN(EC) registration with the College of Nurses of Ontario (with a preferred NP certificate in Primary Health Care or Adult Care)
  • A minimum of 3 years nursing RN experience
  • Working knowledge of Ontario Health atHome role in the community and LTCH sector
  • Critical thinking skills
  • Demonstrated leadership ability to work independently but also as a team
  • Collaborative
  • Empathic and patient/family centred
  • Community/LTC experience and ability to navigate through community/LTC systems is preferred
  • Strong clinical experience preferred
  • Advanced knowledge of MAID legislation and application of professional standards
  • Knowledge in central peripheral and subcutaneous infusion access and administration

What would give you the edge

  • Gerontology Nurse Certified Canada (GNC) is preferred
  • Hospice Palliative Care Nursing Certificate (CHPCN) is preferred
  • Canadian Association of MAID Assessors and Providers (CAMAP) membership is preferred
  • Palliative emergency department mental health/dementia-related behaviours and/or geriatric experience an asset

Hours of work:

Monday to Friday 830am-430pm subject to change as per the collective agreement (35 hours per week; 70 hours per bi-weekly pay period).

Position location and travel:

Hybrid work model subject to change.

Regular travel within the geographical region of Ontario Health atHome Central East may be required. A valid Ontario drivers license and access to a vehicle are necessary.
