Job Description
Job Title: Licensed Practical Nurse (LPN)
Location: Far Rockaway NY
Type: Contract
Shift: Variable Evenings 8-hour shifts 40 hours/week guaranteed
Pay Rate: $36/hour
Job Summary:
We are seeking an experienced and dedicated Licensed Practical Nurse (LPN) to provide high-quality nursing care in a long-term care setting. The LPN will be responsible for administering medications monitoring residents health supporting daily care needs and collaborating with the healthcare team to ensure the best outcomes for residents.
Key Responsibilities:
Administer medications and treatments as prescribed by providers.
Monitor and document residents conditions and report changes promptly.
Assist with wound care clinical procedures and routine assessments.
Collaborate with RNs and the healthcare team on resident care plans.
Support residents with daily living needs and maintain open communication with families.
Requirements:
Active LPN license in New York.
Prior experience in long-term care required.
Strong clinical organizational and communication skills.
