Job Description
Job Title: Physical Therapist (PT)
Location: South Dennis MA
Type: Contract 13 Weeks
Pay Rate: $55 $62 per hour
Position Overview:
We are seeking a dedicated and compassionate Physical Therapist (PT) to join our team on a 13-week contract assignment in South Dennis MA. The PT will play a key role in providing high-quality patient care developing personalized treatment plans and supporting patients in achieving optimal physical function and independence.
Responsibilities:
Perform comprehensive physical therapy evaluations incorporating physiological psychosocial and environmental considerations.
Develop and implement individualized care plans tailored to patient needs.
Adhere to Home Health Resource Group (HHRG) values and maintain compliance with national standards.
Monitor patient progress and ensure annual benchmark outcomes are achieved.
Refer patients to community resources as appropriate.
Update and revise care plans regularly documenting all changes accurately.
Participate in interdisciplinary case conferences to ensure coordinated and effective care.
Supervise and review Home Health Aide care plans every 14 days ensuring proper documentation.
Communicate significant changes in patient conditions to physicians and the care team promptly.
Complete admission and discharge documentation in a timely manner ensuring seamless care transitions.
Demonstrate commitment to service excellence and organizational core values: compassion accountability respect excellence and service.
Qualifications:
Bachelors or Masters degree in Physical Therapy from an accredited institution.
Active Physical Therapist license in Massachusetts.
Minimum of 1 year of clinical experience (home health experience preferred).
Current CPR certification.
Proficiency with electronic health records (EHR) systems is preferred.
