Alfred Health Renal Service provides comprehensive renal care across inpatient outpatient and community settings including dialysis and transplantation. Services span acute and chronic kidney disease and are delivered through multiple sites including Caulfield Sandringham and partner locations like Peninsula Health. Dialysis options include in-centre home and satellite units and transplant services cover deceased live donor and multi-organ transplants.
About the role
The Advanced CKD Nurse Practitioner provides expert clinical support to end-stage renal disease patients guiding treatment decisions. Working autonomously and with the Renal team the NP streamlines assessments referrals prescribing and ongoing reviews across the care continuum.
They manage stage 4/5 CKD cases deliver tailored education and support informed decision-making. The role also develops Renal Supportive Care services for patients choosing conservative care nearing end of life withdrawing from dialysis or experiencing high symptom burden. With advanced expertise the NP enhances care quality and coordination throughout the patient journey.
