RESPONSIBILITIES:
Under the supervision of the Program Director of the Supported Living Program the Case Manager provides client care service documentation and Facility monitoring for clients of assigned apartments and has backup responsibility for other apartments all of which are located in multi-level buildings.
Generally the Case Manager responds to the treatment and rehabilitationneeds of assigned clients monitoring theirprogress stablity and movement throughout the course of treatment in the program. Specifically the case manager must:
A. Assume primary case coordinating responsibilities for all clients that are assigned including but not limited to advocacy counseling crisis intervention and treatment and discharge planning.
B. Orient new clients to the Supported Living Program and facilities or whenever possible enable another client to orient a new client.
C. Develop treatment plans with maximum client participation and support clients efforts to achieve them.
D. Communicate appropriately with other treatment personnel both within and outside the agency exchanging information relevant to client treatment.
E. Conduct intake interviews and case conferences as required to maintain census and coordinate client care.
F. Lead or co-lead group sessions including house meetings on a schedule determined by the Program Director or the Assistant Director.
G. Relay clinical and other client-related or site-related information to other program staff and/or the Program Director or Assistant Director at staff meetings and other appropriate times.
H. Provide services in accordance with all State and federal MediCal requirements and County policies and procedures.
I. Maintain program records including client records progress notes treatment plans and other relevant information in accordance with County policies and procedures and State and federal MediCal requirements.
J. Assist in the training of relief case managers volunteers and interns when requested by the Program Director or Assistant Director.
K. Coordinate and schedule emergency relief coverage when assigned by the Program Director or Assistant Director.
L. Consult with the Program Director the Assistant Director and the Program Consultant as necessary on such issues as case review medications group dynamics etc.
M. Attend and participate in staff meetings supervision sessions and other programmatic and/or administrative meetings as assigned by the Program Director or the Assistant Director.
N. Participate in the on-call system of the program including evenings and weekends.
O. Respond to emergencies in a professional thorough and timely manner. This includes staffing apartment sites as needed to effect 24-hour coverage during a crisis and may further include overnight stays in apartments and scheduling and coordinating relief assistance.
P. Monitor the quality and needs of the program physical environments and assist the Program Director or Assistant Director in the coordination of necessary maintenance and repairs.
Q. Serve as a role model for positive employee relations professional functioning and for adherence to the principles of social rehabilitation.
R. Comply with all applicable federal state and local governmental laws and regulations as well as all agency policies and procedures including the code of ethics.
S. Perform other related duties as assigned by the Program Director the Assistant Director or the Director of Clinical Services.
T. Inform the Program Director or Assistant Director of any personal events and/or circumstances which may affect the ability to perform the proscribed job responsibilities and functions or to meet minimum qualifications.
MINIMUM QUALIFICATIONS:
BA in any field 4 years experience in any mental health related field
MA in any field 2 years experience in any mental health related field
