INTERNSHIP DUTIES:
With direct supervision assists with Transition Specialist duties:
- Administers problem identification sheet/self-assessment for clients who were formerly incarcerated in an effective and accurate manner.
- Identifies clients needs including but not limited to: housing substance abuse treatment mental health services medical services mentoring services family assistance and services counseling employment training adult basic education or other educational needs.
- Coordinates with services providers to provide linkages for services for identified client needs in an effective efficient and courteous manner. Accurately documents information for client case file. Completes appropriate release of information forms as required for making a referral and/or releasing information.
- Stays informed of community resources. Makes referrals to community organizations for clients. Follows up with clients to ensure services were provided. Intervenes and acts as an advocate for clients when problems or issues arise as needed.
- Provides exceptional customer-service to clients and contribute to a team-focused productive positive and therapeutic environment.
- Assists clients with becoming more positive productive prosocial members in the community.
- Participates in team meetings as scheduled with Program Services Coordinator. Discusses client issues and case reviews.
- Attends Community Events as a representative of North Star NRRC.
- Performs other duties as assigned.
