Erie Family Health Centers a top workplace in Chicago and suburbs is looking for a valuable addition to our Advanced Practice Provider Team! The Urgent Care Primary Care Advanced Practice Provider or Physician will provide comprehensive primary health care and urgent care services to patients. Performs physical examinations on new and return clients. Refers patients to specialists as necessary. During peak viral season provides urgent care services with a special focus on pediatric patients. Conducts care of newborns hospital discharge patients and pre-operative patients.

*Compensation will vary depending on specialty and is based on each candidates experience skills and education within the range identified for the role. Candidates who meet the minimum requirements of the role will start at entry in the range. Any additional skills experience and education will be reflected in the compensation offered.

Main Duties & Responsibilities

  • Performs history and physical on new/return patients throughout the lifespan.
  • Assess plan and treat patient needs within an urgent care setting and triage patients to differentiate between immediate and less urgent care.
  • Assess plan and treat patients
  • Order labs tests injections referrals and prescribe medications.
  • Order collect and perform appropriate laboratory and diagnostic studies.
  • Delegate work appropriately to other caregivers and ensures accuracy of delegated work.

Qualifications

Education

  • M.S. in Physician Assistant Studies MSN or DNP from accredited nursing school OR MD/DO
  • For MD/DO candidates - Completion of residency training in Family Medicine or Pediatrics and Board certified or Board Eligible in Family Medicine or Pediatrics.
  • For Advanced Practice Providers - Active PA or APRN license in Illinois and Board Certified. Must maintain an active collaboration practice agreement with an Erie Physician if does not have Full Practice Authority.
  • Active DEA and controlled substance License
  • Active CPR Certification

Experience

  • Minimum of two years of experience as Urgent Care/ Family practice and/or primary care
  • Preferred experience serving in FQHC Community Health Centers or traditionally unserved patient populations
  • Preferred Medical Spanish experience or highly motivated to learn to utilize interpreters for patient encounters in foreign languages

Skills and Knowledge

  • Commitment to increasing number of APRNs working in underserved communities and providing high-quality compassionate patient care.
  • Comfort with providing clear and constructive feedback
  • Energetic deadline-driven self-starter
  • Excellent organizational and project management skills
  • Working knowledge of PC using Microsoft products such as Word and Power Point in a Windows operating environment
  • Ability to develop and maintain excellent working relationships with preceptors partners clinical staff managers and directors.
  • Interest in and comfort with working with diverse patient population
  • Knowledge of or interest in learning medical Spanish
