General Responsibilities

Reports to the Behavioral Health Program Manager. The Behavioral Health Specialist is responsible for delivering high quality client-centered support to individuals experiencing mental health emotional and/or substance use challenges. This position involves direct care services treatment planning support crisis intervention and coordination of behavioral health services in accordance with best practices and regulatory standards. The Behavioral Health Specialist works as part of an interdisciplinary team to improve outcomes and quality of life for clients across a variety of settings.

Duties

  • Support Behavioral Health Therapists by ensuring case notes are updated promptly accurately and in compliance with organizational policies and funding requirements.
  • Assist in developing and updating Individual Service Plans (ISPs) in collaboration with therapists clients and interdisciplinary team members ensuring goals are person-centered and measurable.
  • Gather review and organize clinical information from assessments screenings and collateral sources to support diagnostic impressions.
  • Contribute to developing diagnostic impressions by synthesizing client history presenting concerns behavioral observations and assessment data under the guidance of a licensed clinician.
  • Monitor client progress toward ISP goals documenting changes and recommending adjustments in coordination with the treatment team.
  • Coordinate with case managers peer support specialists and medical providers to ensure continuity of care and integration of behavioral health services with other supports.
  • Maintain confidentiality and compliance with HIPAA agency protocols and ethical standards in all client documentation and interactions.
  • Participate in team meetings and case conferences to discuss client needs progress and service adjustments.
  • Provide client education on behavioral health resources treatment options and wellness strategies as directed by licensed staff.
  • Assist with data collection and reporting for program evaluation and quality improvement purposes.
  • Conduct behavioral health screenings intake assessments and psychosocial evaluations to identify client needs and risk factors.
  • Assist in developing implementing and updating individualized treatment or recovery plans in collaborations with clinical staff.
  • Provide supportive counseling psychoeducation and life-skills coaching to promote client self-sufficiency and recovery.
  • Respond to behavioral health crises including performing bask risk assessments and assisting with de-escalation under supervision.
  • Coordinate referrals and services with external providers community agencies and internal care teams to ensure continuity of care.
  • Support client navigation of healthcare housing employment benefits and legal systems by providing referrals and follow-up assistance.
  • Maintain timely and accurate clinical documentation in compliance with organizational policies HIPAA and accreditation standards (e.g. CARF Medicaid and DATEP).
  • Participate in multidisciplinary case reviews team meetings and ongoing professional development opportunities.
  • Assist with community outreach efforts including attending health fairs community events and client engagement activities.
  • Track and report client progress and outcomes to support quality improvement and grant reporting requirements.
  • Monitor attendance participation and follow-up for behavioral health groups and individual sessions.
  • Provide administrative and logistical support to behavioral health programs including scheduling appointments and coordinating transportation resources when needed.

Employment Standard & Conditions


EXPERIENCE: Bachelors Degree in Psychology Social Work Counseling Behavioral Science or related field required. Minimum of 1-3 years of case management addition the incumbent must have a minimum of two years of experience in a behavioral health or human services setting. Experience working with diverse populations and understanding of trauma-informed care preferred.


Knowledge Skills & Abilities

  • Must be able to work as an integral part of the BH team and exhibit team building skills and competencies.
  • Must be extremely familiar with social services resources offered in the Atlanta Metropolitan Area and across the State of Georgia.
  • Must possess outstanding organizational time management and communication skills.
  • Must demonstrate solid decision making skills.
  • Must have strong interpersonal skills and possess the ability to interact effectively among a multifaceted multidisciplinary team.
  • Must communicate effectively in writing as appropriate to document and record client service plans goals and progress.
  • Must have excellent oral and presentation skills to facilitate access to services and convey information effectively with staff clients physicians behavioral specialists and other service providers.
  • Demonstrated experience working with diverse medically underserved high risk populations.
  • Must be culturally competent.
  • Must have strong computer skills.
  • Must be able to work flexible hours when required.

Status:Full-Time.

Job Location:The position is based in Atlanta and requires work at the ERC office(s).

SALARY RANGE:Salary is contingent upon available funding and is commensurate with experience and qualifications.
