POSITION DESCRIPTION:

The Day Hab Community Connections Coordinator is part of TNCs Community Connections Team which ensures all daily activities are planned/carried out to support the safety health and personal interests of the individuals in TNC Communitys care. If you want to make a meaningful difference for people with special needs serving as a DCCC is a great opportunity to help individuals with intellectual and developmental disabilities get involved in their community try new things and live fulfilling lives. DCCCs will: provide day habilitation services to adults with developmental disabilities including classroom activities in exercise and fitness crafts leisure and daily living skills. Includes feeding and personal hygiene assistance. There will also be opportunities to assist with community off-site activities. DCCCs advocate for the removal of architectural environmental attitudinal financial employment communication transportation community integration and other barriers for individuals receiving services through TNC Community. DCCCs are mandatory reporters of all suspected/alleged abuse and neglect in accordance with TNC policies.

III. PROGRAM RESPONSIBILITY/ESSENTIAL FUNCTIONS:

  1. Identify activities for each individual that align with the interests and goals outlined in their current Individualized Support Plan including but not limited to: classes and hobbies as well as opportunities to try new things and learn/develop skills. Emphasize selecting activities that broaden the individuals social network through participation with people.

  1. Encourage each individuals enthusiasm/interest to participate in activities and facilitate their interpersonal connections with others.

  1. Acquire consent ensure accessibility register schedule and facilitate/support/remain actively involved throughout individuals participation in enrichment activities. Most TNC individuals require side-by-side hands-on support throughout activities due to mobility communication and/or medical needs.

  1. Assist with the grooming needs of the individuals including but not limited to: tooth brushing hand washing feeding/lunchtime clean up etc.; always promoting the independence of the individual.
  2. Participate with and assist individuals in reaching their ISP goals by assisting in assigned classroom or on outings or rotating with an assigned individual/student throughout the day as needed and requested.
  3. Work with your leadership to determine staffing requirements to support individuals in their activities to maintain appropriate staff-to-individual ratios.

  1. Maintain and post activity calendars with accurate dates times and activity descriptions for individual and staff member reference.

  1. Facilitate the program schedule/routine.

  1. Follow and implement supervisory directions.

  1. Use appropriate behavior management techniques; assist and cooperate in implementing Individual Support Plan (ISP).

  1. Document each individuals activities for reports (including case notes and weekly or monthly summaries records of time specific activities and transportation) and promotion/marketing through written descriptions photos highlight details and quotes. Submit receipts for spending from individuals funds and organizational funds.

  1. Work with Development Director Marketing Manager and Community Connections Specialist to share stories through TNC Community social media website and other communications with attention to maintaining individuals rights to privacy and dignity.

  1. Understand and implement behavior modification principles that reinforce desirable behaviors. Never use punishment or abusive language or behavior.

  1. Serve as a positive role model who encourages individual independence and awareness.

  1. Submit medical information/records when required into electronic documentation system (Therap). Update notes in Therap daily.

  1. Communicate regularly with Community Connections Program Manager Chief Operating Officer and Development Director regarding individuals/guardians satisfaction with activities.

  1. Request and maintain equipment/supplies needed for individuals and Day Habilitation Program.

  1. Maintain individual and program confidentiality and uphold individuals rights. Do not discuss identifiable individual information about supported individuals or agency personnel in the presence of others.

IV. HEALTH AND SAFETY RESPONSIBILITY/ESSENTIAL FUNCTIONS:

  1. Provide TNC transportation for individuals to and from scheduled activities.

  1. DCCC will be familiar with the individuals medical/medication/insurance information and ensure medications are administered as prescribed when on outings with individuals.

  1. Report identified health issues to Director of Health Services (DHS) and Community RN. Monitor health issues as identified and/or directed by DHS/Community RN. Document

status of health issues by entering a T-Log in Therap after each contact. (For life-threatening emergencies call 9-1-1 FIRST)

V. PERSONNEL RESPONSIBILITY/ESSENTIAL FUNCTIONS:

  1. DCCC work schedule is Monday-Friday 7:30am-4:00pm.

  1. Respond to activity-related issues at assigned sites.

  1. Provide on-call support for activities as needed for entire agency.

  1. No supervisory responsibilities.

VI. FINANCIAL RESPONSIBILITY/ESSENTIAL FUNCTIONS:

  1. Complete mileage/transportation logs and submit them to the Chief Financial Officer by the last day of the month.

  1. Submit receipts for gas and other purchase(s) to the Chief Financial Officer by the last day of the month in which the purchases are made.

  1. Reasonable/customary/approved DCCC activity expenses are reimbursed after submission of appropriate receipts.

  1. Submit activity-related bills/invoices in a timely manner to the Chief Financial Officer for processing.

VII. FACILITY/TRANSPORTATION RESPONSIBILITY/ESSENTIAL FUNCTIONS:

  1. Assigned DCCC vans are to be used first with other transportation as required. New activities must be posted as soon as possible to allow time for transportation planning.

  1. Maintain TNC Community vehicles in a neat organized clean and safe manner. Leave vehicles with a minimum of tank of fuel after each use.

  1. Report identified service needs on vehicles by completing service repair form immediately after use.

VIII. OTHER RESPONSIBILTIES:

  1. Attending regularly scheduled Community Connections Team meetings all-staff meetings and in-service training/education.

  1. Complete/file forms paperwork and reports according to guidelines in a timely manner.

  1. Assist in maintaining an orderly safe and hygienic environment for individuals and yourself.

  1. Other related duties as assigned.

IX. MINIMUM EDUCATION AND EXPERIENCE QUALIFICATIONS:

Must be at least 18 years of age

High School Diploma or GED required

Minimum 2 years of experience in social services

X. PREFERRED EDUCATION AND EXPERIENCE QUALIFICATIONS:

Associate degree or higher or equivalent experience

CNA or CMT Certification

Experience working with IDD individuals in a professional context

Experience planning/facilitating community activities

XI. COMMUNICATIONS:

The position requires proficiency in English sufficient written verbal and nonverbal communication skills to effectively engage with individuals staff volunteers community members and administration.

XII. ADDITIONAL REQUIREMENTS:

Ability to clear background checks and drug/alcohol screenings as required

Current CPR/First Aid certification
Maintain current Abuse/Neglect Training

Completion of Gentle Teaching or Equivalent

Must participate in all mandatory trainings and re-certifications as required

Ability to drive motorized vehicles

Valid class E Missouri drivers license or equivalent

Must have own transportation and proof of insurance (may not be required)

Ability to lift 75 pounds (weight may vary)

Flexible schedule

Familiar with and able to use a computer

Other duties as assigned

XIII. PHYSICAL EXPECTATIONS:

These physical demands are representative of the physical requirements necessary for an employee to successfully perform the essential functions of the Day Hab Community Connections Coordinator position. While performing the responsibilities of the Day Hab Community Connections Coordinator the employee is required to talk and listen. Employees are often required to sit and use his/her hands and fingers to handle or feel drive type and file. Employees are often required to stand walk reach with arms and hands climb or balance and stoop kneel crouch or crawl. He/she lifts and moves items and/or persons up to 75 pounds and greater weights using a lift devise or two-person lift/transfer. Vision abilities required by this position include close vision.
