Job Description

Position Title: Occupational Therapist Homecare

Location: Orlando FL
Payrate: $46 $56/hr

Position Summary:

The Occupational Therapist (OT) is a licensed healthcare professional responsible for providing skilled medically necessary occupational therapy services in the home setting in accordance with physician orders regulations and applicable guidelines. The OT evaluates patients functional abilities develops and implements individualized treatment plans and provides interventions to improve independence in activities of daily living (ADLs) instrumental ADLs (IADLs) safety and participation in meaningful occupations. The OT ensures all documentation supports medical necessity and skilled needs and meets regulatory standards.

Essential Duties and Responsibilities:

Evaluation and Plan of Care

  • Conduct comprehensive initial evaluations including assessment of ADLs IADLs cognition coordination upper extremity function visual-perceptual skills sensation pain and home safety.
  • Develop establish and periodically review an Occupational Therapy Plan of Care (POC) with measurable objective short- and long-term goals.
  • Obtain physician certification/recertification for all POCs to ensure compliance with relevant guidelines.

Treatment and Skilled Interventions

  • Provide skilled interventions such as therapeutic activities ADL retraining fine/gross motor coordination cognitive retraining energy conservation joint protection home safety education and adaptive equipment training.
  • Recommend and train patients/caregivers in the use of durable medical equipment (DME) and adaptive devices to improve independence and safety.
  • Educate patients/caregivers on fall prevention ergonomics joint protection and techniques for safe performance of daily activities.
  • Modify and progress treatment plans in response to patient performance safety and recommendations.
  • Assist in discharge planning including recommendations for continued care or equipment needs upon discharge.

Documentation and Compliance

  • Complete all documentation (evaluations visit notes progress notes recertifications and discharge summaries) within required timeframes.
  • Ensure all records meet regulatory standards clearly supporting skilled need medical necessity and ongoing progress.
  • Document functional progress using standardized outcome measures (e.g. AM-PAC 6-Clicks Barthel Index FIM DASH).
  • Participate in internal audits and compliance training.

Interdisciplinary Communication and Teamwork

  • Collaborate with physicians nurses physical therapists speech therapists and other healthcare professionals to ensure coordinated patient-centered care.
  • Actively participate in case conferences team meetings and interdisciplinary communication.
  • Report significant changes in patient condition to the physician and relevant parties promptly.
Qualifications:
  • Graduate of an accredited Occupational Therapy program (MOT OTD or equivalent).
  • Current active Occupational Therapist license.
  • Minimum 1 year of clinical experience preferred; home health experience strongly desired.
  • Knowledge of relevant regulations and guidelines.
  • Current CPR/BLS certification.
  • Valid drivers license reliable vehicle and auto insurance (as required).
Knowledge Skills and Abilities:
  • Ability to perform accurate evaluations of functional and cognitive deficits and design measurable patient-specific goals.
  • Knowledge of home health regulations and OT-specific compliance requirements.
  • Clinical expertise in ADL retraining functional cognition safety strategies and adaptive equipment use.
  • Strong communication and teaching skills to train patients/caregivers effectively.
  • Ability to work independently while maintaining accountability and compliance with standards in a homecare setting.
