Under the direction of the Program Manager Community Health Workers are responsible for conducting outreach and education activities for identified populations. Community Health Workers are responsible for identifying possible outreach and education locations and effective strategies to reach target population. Community Health Workers will lead education sessions to both groups and individuals. Community Health Workers working under this program will need to adhere to established procedures for outreach education advocacy and client referral. The focus area for Community Health Workers will vary according to program goals. Community Health Workers must become knowledgeable of focus area content approved messaging and required procedures to advocate for clients. Community Health Workers will need to adhere to established procedures for outreach educations advocacy and client referral.
