Fully accredited by The Joint Commission we offer a full range of services including 24/7/365 Emergency Department and Level IV Trauma Center hospitalist program testing surgery orthopedics rehabilitation and more so that you can experience better health.

Job Summary and Responsibilities

As a Hospice Home Health Aide you are more than a caregiver you are a cornerstone of comfort providing essential support that allows our patients to live their final chapters with dignity in their own homes.

As a Hospice Home Health Aide you will provide essential hands-on care ensuring dignity and well-being for our patients. Under the guidance of a Registered Nurse your daily impact will include:

  • Personal Care & Comfort:Assisting with personal hygiene mobility (ambulation and transfers) and promoting overall patient well-being.
  • Patient Monitoring: Diligently reporting any changes in condition to the nursing team.
  • Supportive Environment:Performing light housekeeping preparing meals and ensuring a safe healthful and comforting home environment.

Job Requirements

  • High School Diploma or equivalent required.
  • Certified Nurse Assistant (CNA) in the state of Minnesota (MN) required.
  • Basic Life Support (BLS) required.
  • Must have a valid Drivers License.
  • Must have current vehicle insurance.
