We are seeking a Physical TherapistPRNwhose primary responsible for their direct clinical skills documentation skills and the compliance to all professional standards.

Key Responsibilities:
Responsible for the evaluation and treatment of residents who demonstrate disorders of musculoskeletal deficiencies neurological conditions balance disorders and transfer/gait deficits. Evaluation and treatment will only be conducted after a written order is received from the physician.
- Responsible for the screening of residents of the community as follows: New Community residents: Within 24 hours of admission Long term residents: Upon receipt of a significant change form and/or upon referral from nursing.
- Responsible for attending all required community meetings and in-services including Medicare meetings family meetings care plans
QUALIFICATIONS:
Minimum Education Required:
Masters of Science Degree or Bachelor of Science Degree in Physical Therapy. State Licensure.
Experience:
As applicable to assignment
