- 32nd Street Outpatient Pharmacy Phoenix AZ 85012
- Note: The first two sites are on the main campus; the third site is one lane away from the main campus.
Duration: 1 year contract with the possibility of a 4 year extension
Shift Schedule: Facility operates 24/7.
- Pharmacists may be scheduled for:
- 8 11 or 12-hour shifts
- Day: 06 AM 02:30 PM
- Evening: 2 PM 10:30 PM
- Overnight: 7 PM 06 AM or 8 PM 6 AM
- Weekend and holiday shifts may be required based on staffing needs.
License/Certifications:
- Unrestricted license to practice pharmacy in any U.S. State Territory or the District of Columbia.
- Must be licensed to handle controlled substances.
- No history of license restrictions suspensions or revocations.
- Must pass credentialing and privileging requirements via VetPro (60-day process).
Education:
- Graduate of an ACPE-accredited College or School of Pharmacy.
- Minimum degree: B.S. in Pharmacy or Doctor of Pharmacy (Pharm.D.)
Minimum Qualification/Experience:
- 24 months of relevant experience within the past 36 months preferred.
Responsibilities:
- Deliver full-spectrum pharmacy services per VA standards.
- Dispense prescribed medications and advise patients on proper usage.
- Evaluate medication orders for:
- Drug interactions
- Dosing appropriateness
- Allergies
- Duplications and contraindications
- Counsel patients regarding adverse effects and drug interactions.
- Recommend therapeutic alternatives aligned with VA formulary.
- Ensure compliance with state and federal drug control regulations.
- Maintain medication security and inventory control.
- Input and verify prescriptions in the VAs electronic medical record (VistA/CPRS).
- Report outcomes and participate in continuous quality improvement initiatives.
- Collaborate with healthcare providers to enhance patient care and medication safety.
- Maintain professional and cooperative interactions with patients providers and staff.
