Responsibilities: Provide skilled nursing care and health guidance to individuals and families in the home and community. Conduct comprehensive assessments including physical psychosocial cultural and environmental needs. Develop and implement individualized care plans and adjust them based on patient progress. Coordinate care with interdisciplinary team members including physicians therapists and aides. Educate patients and families on disease processes self-care prevention and ethical concerns. Provide symptom management and specialized nursing interventions including for hospice patients. Maintain accurate and timely clinical documentation. Participate in team conferences quality assurance activities and performance improvement initiatives. Serve as a mentor and resource to other staff patients and caregivers. Participate in an on-call rotation as required. Qualifications: Current Vermont Registered Nurse (RN) license required; BSN preferred. Minimum of one year of clinical nursing experience (community nursing or related field preferred). Must have home health experience. Valid driver s license and reliable transportation required. Strong computer proficiency and ability to maintain electronic health records. Excellent communication organizational and problem-solving skills. Ability to work independently and collaboratively in a dynamic community health environment. Must have experience with Oasis.
