The Colorado Mental Health Hospital in Fort Logan (CMHHIFL) is one of two state hospitals serving our community. The vision of CMHHIFL is to provide the highest quality mental health services to persons with complex serious and persistent mental illness within the resources available. Our mission is to support those individuals in achieving their recovery goals and reaching their full potential. CMHHIFL provides inpatient treatment to adult patients generally between the ages of 18-59. The hospital currently has 138 adult inpatient beds and over six treatment milieus serving civil and forensic individuals. Medical services occupational therapy neuropsychological-rehabilitation services and trauma-informed care services are available for all patients throughout the hospital.

About this Work Unit:

The Social Work III position works in the 24-hour State psychiatric hospital with individuals voluntarily and involuntarily committed for treatment. Patients receiving treatment at CMHI-FL have severe and persistent mental illness and may have traumatic brain injury trauma disorders and other complex dynamics. Each treatment team is multidisciplinary including psychiatrists psychologists social workers addiction specialists occupational therapists recreation therapists creative arts therapists nurses physicians and mental health addition the hospital offers dietary peer and chaplain services

Description of Job

About the Position:

The Social Work unit collaborates with patients their families and/or support systems and external stakeholders to plan organize and execute psychiatric treatment interventions and comprehensive aftercare plans with placement. Services include clinical assessment treatment planning individual therapy group therapy family therapy counseling and education. This work unit serves as a liaison between the hospital and Community Mental Health Centers (CMHCs) and other governmental entities.

The purpose of the Social Worker is to provide individual group and family therapy. Individual and family therapy are provided as needed which is determined in the treatment plan meetings and collaboration with the treatment team. Group therapy is offered to all patients and group assignments shall be determined by clinical and individualized need. The Social Work positions are responsible for developing group materials appropriate for patient-specific needs and leading group therapy sessions. The Social Worker completes comprehensive biopsychosocial assessments and reassessments coordinates treatment planning for all patients and regularly documents in the electronic health record. The Social work team is responsible for the coordination of discharge and comprehensive aftercare doing so this position collaborates with support systems stakeholders CMHCs and other agencies to coordinate continuity of care. The Social Worker actively contributes to hospital-wide events in-service training and program development.

The Social Worker III provides supervision to the social work externs in the externship training program and supports execution of the hospitals mission vision and values within the social work department and in collaboration with leadership. As a student supervisor the Social Worker III is responsible for clinical and administrative supervision as well as performance management and disciplinary processes. The Social Worker III provides autonomous clinical service provision by ensuring their work product contributions are reviewed complete and timely. This includes treatment plans documentation assessments and therapeutic interventions. This position is also responsible for contributing to the implementation of various departmental and discipline-specific projects and supporting implementation within the team. Duties in this position include:

Assessment Intervention and Treatment Planning:


Case Management and Discharge Planning:
  • Responsible for their own treatment caseload and is required to ensure all coordination of resources is complete in a timely manner. This may include ensuring submission of benefits identification of appropriate benefits guardianship needs Momentum referrals obtaining identification securing a payee applying for Medicaid/Medicare and submitting referrals to potential discharge locations.
  • Must work closely with the Benefits Specialist and Reintegration Specialist and assist in completion and monitoring of such documents and referrals and may collaborate with legally appointed guardians or members of the patients support system to obtain the necessary information and navigate the process.
  • Identifies appropriate discharge locations in collaboration with the treatment team and liaisons with those placements to support continuity of care.
  • Coordinates day passes off-campus attends placement visits coordinates family and other visits and attends other off-campus appointments as needed.
  • Attends interagency staff meetings often and is responsible for providing education to stakeholders regarding the Social Work Department and building relationships across various agencies.
  • Supports team members in navigating various systemic barriers and ensuring timely and accurate processes are completed. All efforts are geared toward supporting discharge into the least restrictive environment.

Specialty Program and Project Implementation:
  • Collaborates with their primary supervisor to support implementing designated specialty programs and/or departmental projects. This may include specialized treatment modalities including but not limited to Animal Assisted Therapy Substance Addiction Treatment Virtual Reality programs and Peer Programs.
  • Partnering with community stakeholders for shared resources and development of discipline-specific training opportunities. At times these duties require liaising with community stakeholders community site visits creating and sharing information materials or sessions establishing protocol and policy training staff and overseeing and auditing the implementation and outcome measures.
Student Training and Student Supervision:
  • CMHHFL serves as a training site for social work externship programs. The Social Work Department is responsible for interviewing and selecting social work intern level students.
  • Contributes to training opportunities through externship level didactic curriculum and supervision. As the students supervisor this position is responsible for fostering skill development ensuring training opportunities assessing performance providing critical feedback and supporting successful completion of all internship requirements.
  • Receives supervision from their primary social work supervisor.
Other Duties as Assigned:
  • Other duties as assigned include hospital-wide ad hoc activities milieu support etc.

Education/Licensure/Certification/Experience:

Current valid Colorado or a State participating in the Social Work Licensure Compact licensure as:

And one (1) year of licensed experience based on the work assigned to the position.


Preferred Qualifications:

  • Social Work degree and experience

  • Inpatient Psychiatric Treatment

  • Treatment for adults with severe and persistent mental illness

  • Forensic and/or Competency Restoration Program experience

  • Previous State Employee


Conditions of Employment:

The successful candidate in this position must be willing and available to submit to the following conditions:

  • Must possess a valid unrestricted U.S. Drivers License
  • Incumbents of this position must be at least 21 years of age.
  • Must be able to use handle or be exposed to hazardous material such as bodily fluids
  • May be able to travel to attend training court hearings transport patients to appointments and other related activities.
  • TB testing annual influenza and other vaccines as required by the State of Colorado
  • Former State employees who were disciplinarily terminated or resigned in lieu of termination must:
    • Disclose that information on the application.

    • Provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position.

    • Provide the employee number from the applicants prior State employment. Absent extraordinary circumstances prior disciplinary termination or resignation in lieu of termination and failure to provide this information will disqualify the applicant from future State employment with CDHS.
