Job Description

OCES supports elders and individuals with disabilities by providing vital information and coordination of services. By promoting healthy safe lives for our consumers we hope to allow them to stay as active and engaged in their lives and in their own communities for as long as possible.

We are seeking a Community Transition Liaison to facilitate successful discharge activities and other support activities for assigned consumers focusing on the transition from nursing facilities (NF) to community.

Position is on-site at various nursing facilities.

What you will do:

  • You will be the point of contact for families health care proxy and nursing facility staff related to transitions to the community.
  • Increase awareness of service and transitions options with residents and community.
  • Determine appropriateness for referral programs supporting the transition to community and in support of meeting consumer needs once in the community setting.
  • Participate in Interdisciplinary Discharge Planning (IDP) meetings and facilitate communication among consumer family member and community agencies.
  • Maintain records in compliance with standards.
  • Maintain knowledge of current OCES policies/standards and funding source regulations.

What you will bring:

  • Bachelors Degree in nursing social work human services or related field required.
  • Current Social Work License preferred.
  • Excellent interpersonal and conflict resolution skills.
  • Must possess knowledge of long-term care case management discharge planning community resources programs and benefits to help support an individuals transition from an institutional to a community setting
  • Two years of experience in the social services field including a minimum of one year of experience with transition support activities
