Position: Clinical Nurse Specialist Geriatric Behavioural Consultant Virtual Behavioural Medicine Program 
Site: University Centre Toronto Rehab 
Department: Geriatric Psychiatry 
Reports to:Manager Program Services 
Hours: Days 1.0 FTE 
Status: Temporary Full-Time(approximately 7 months)
Closing Date: September 10 2025

University Health Network (UHN)is looking for an Advanced Practice Nurse to work in the Virtual Behavioural Medicine Program part of the Geriatric Psychiatry Services at Toronto Rehab.  This is a virtual service supporting people with dementia and responsive behaviours primarily in long-term care homes across Ontario.   

In this role theAdvanced Practice Nurse (APN) functions in an expanded specialized role with a high level of autonomy and expert skill to formulate clinical decisions and support the management of behavioural and psychological symptoms and promote quality of life for people with dementia.   

The principal responsibilities include: demonstrating a comprehensive theoretical knowledge base and advanced level of clinical competence in caring for complex patients involving collecting and interpreting data about the health and behaviours of the patient to develop a pharmacological and non-pharmacological care plan in collaboration with geriatric psychiatry; working collaboratively with geriatric psychiatrists and a multidisciplinary mental health team; playing a leadership role and supporting quality improvement and program evaluation; participating in policy and procedure development; participating in education and professional development activities/programs; performing activities to further develop and strengthen the profession; acting as a resource and serving as a consultant to individuals and groups within the professional community and other hospital/agencies; performing cross-functional and/or other duties consistent with the job classification with potential career growth and mobility across UHN. 


Qualifications :

  • Masters or Doctorate in Nursing with specialization in Geriatrics Mental Health or equivalent. 
  •  Registered Nurse (Extended Class) in good standing with the College of Nurses of Ontario. 
  •  Minimum 5 years of clinical experience in geriatric mental health or behavioural health. 
  • Expertise and/or certification in management of behavioural and psychological symptoms of dementia 
  • Confidence in the care of medically complex older adults with multiple co-morbidities in particular people living with dementia and residents of long-term care 
  • Experience in nursing research program planning and evaluation methodologies 
  • Comfortable working in a virtual environment 
  • Ability to perform duties in a professional and courteous manner and produce high quality work while meeting deadlines in accordance to UHN standards 
  • Exercise initiative and good judgment with ability to multi-task 
  • Effective organizational interpersonal and communication skills 
  • Client-service oriented with the ability to effectively work with diversity and appreciate that people with different opinions backgrounds and characteristics bring richness to the challenge or situation at hand 
  • Excellent time management skills 
  • Excellent decision-making problem recognition and problem-solving skills 
  • Excellent conflict resolution skills 
  • Commitment to collaborative practice 
  • Ability to work effectively independently and under stressful conditions. 
  • Ability to utilize information technology 

Preferred Qualifications: 

  • Certification in Gerontological Nursing or Psychiatric/Mental Health Nursing. 
  • Experience in academic health science centres or integrated care models. 
  • Knowledge of Ontarios Behavioural Supports Ontario (BSO) framework.
