You will be updated with latest job alerts via emailAct as a duty worker 5 days per week responding to urgent referrals and new cases within Adult Mental Health.
Provide timely risk assessments crisis interventions and signposting or allocation for further intervention.
Prioritise cases effectively under pressure balancing urgent and routine responsibilities.
Manage a complex caseload of adults with severe and enduring mental health conditions.
Undertake Care Act assessments mental capacity assessments and other statutory duties under relevant legislation.
Work within the framework of the Mental Health Act Mental Capacity Act and Section 117 aftercare.
Attend and contribute to ward rounds CPA meetings and Mental Health Tribunals.
Collaborate with housing teams brokerage services and clinical professionals to coordinate care packages and accommodation.
Liaise with legal representatives court staff and safeguarding partners as needed.
Undertake safeguarding enquiries in line with the Care Act and local procedures.
Prepare documentation for and attend Court of Protection hearings as necessary.
Ensure adherence to statutory duties including accurate and timely record-keeping.
Travel within and outside the borough to complete home visits attend hospital discharge planning meetings and represent the team at external settings.
Qualified Social Worker (e.g. BA/BSc/Master s in Social Work DipSW CQSW).
Registered with Social Work England.
Minimum of 2 3 years experience working in adult mental health services.
Proven ability to manage complex mental health cases independently.
Experience with Section 117 Court of Protection Mental Health Tribunals and multi-disciplinary working.
Skilled in prioritising under pressure and managing high workloads.
Experience conducting safeguarding enquiries and statutory assessments under the Care Act 2014.
Full UK driving licence and access to own vehicle.
