Position Summary:
The Senior Project Manager is a key leader within the Connected Care portfolio reporting to the Project Director. Their initial area of focus will be implementation of a Central Intake model for referrals across the Toronto Region in close collaboration with Ontario Health. This work supports the broader strategic vision of advancing integrated and patient-centered care across the continuum.
The Senior Project Manager will lead a project team and work closely with healthcare providers system administrators and interest holders across the region. They will facilitate working groups steering committees and advisory bodies to co-design and implement innovative models of care enabled by digital solutions.
This position plays a critical role in shaping and executing transformative initiatives that span local regional and provincial levels. Success in this role requires a strong health system lens the ability to mobilize cross-sector collaboration and expertise in project and change management. The Senior Project Manager will ensure that Connected Care initiatives are delivered effectively sustainably and in alignment with UHNs strategic goals and values.
