Description

UCP of Central Arizona is a nonprofit organization whose mission is to provide comprehensive services to individuals with disabilities and their families. UCP of Central Arizona offers adult day center employment services and home and community-based services. The Adult Program Support Specialist is the primary support person for the day-to-day administration and support coordination of the UCP Adult programs in a center-based work environment.

Essential Functions:

Member File Maintenance:

  • Monitor Focus Service Notifications/Potential Members.
  • Complete the referral process for new member matches within the required timelines.
  • Ensure completion of all intake materials for member files within the required timelines.
  • Create member files for new members and enter them into UCP databases.
  • Prepare and enter supporting materials into member files and provide a copy to DDD.

Program Support

  • In support of the adult program leadership provide program support to meet the needs of the adult programs.
  • Enter data into AHCCCS database on HCBS workforce certificates
  • Support data gathering for scorecard dashboard and metric reporting for adult programs.
  • In conjunction with Program supervisor support and assist with compliance training scheduling and tracking for program employees.
  • Provide data and analytics to support Adult Program leadership in informed decision-making.
  • Promptly act as the first point of contact for individuals interested in UCP adult programs facilitating the intake process and reviewing the process for continuous improvement
  • Attend expos job fairs and any other community events to share information about UCP adult programs.
  • Must be able to provide own transportation to and from provider meetings and community engagements.
  • Support program management with provider communications gathering of department required materials and program processes and protocols required for operations.
  • Document standard operating procedures for UCP adult programs
  • Provide reports regarding intake information new provider new client and prospects and submit to program leadership.
  • Protect and maintain the confidentiality of member information and documentation in accordance with UCP policy HIPAA state and federal law.
  • Support program management with administrative tasks as requested.
  • Provide in-program coverage support when scheduled and as required.
  • Model UCPs mission.
  • Perform other duties as assigned.

Qualifications

  • Strong Communication Skills- verbal and written.
  • Knowledge of HCBS Regulations and Policy. Knowledge of the Disability community and experience serving that community.
  • Empathy and Advocacy are the fundamentals. Commitment to improving the quality of care for those receiving HCBS services without judgement.
  • Strong Excel knowledge and other systems Word SpokeChoice Power Point a plus EVV and able to have strong organizational skills.
  • Problem Solving trouble shooting day to day issues possess critical thinking skills with the ability to think creatively to approach solutions.

Qualifications

Minimum Education and Experience:

  • High School Diploma or equivalent.
  • Minimum of one (1) year of experience supporting individuals with developmental disabilities
  • Must have at least one year of verifiable experience working in the DDD community or more than one year as a direct care professional providing support services to individuals who are developmentally or physically disabled.
  • For assignments that require the employee to drive a vehicle have a valid state drivers license and meet the minimum requirements outlined in the companys Transportation Policy.
  • Experience working with children and adult services that support individuals with a variety of disabilities.
  • Demonstrated ability to productively collaborate with clients co-workers external constituents committees and UCP Managers.

Preferred Education:

  • Associate or Bachelors level degree preferred in social services human services program management and/or two (2) to three (3) years of combined experience in a support or liaison role within developmental disabilities.
  • Bilingual in Spanish preferred

Required Knowledge Abilities and Skills:

  • Current knowledge of Arizona DDD systems.
  • Ability to decipher and analyze data to support the mission and vision of the organization.
  • Must be proficient in software systems for program management such as Spoke Choice Focus other comparable systems.
  • Must be proficient using Microsoft Word Excel PowerPoint and Outlook and DDD applications.
  • Office Equipment fax machines copiers scanners phones etc.
  • Excellent telephone & interpersonal skills.
  • Must be able to function independently with little to no supervision.
  • Must have time management skills with the ability to work under deadlines and multiple projects.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must be flexible and support the team as needs arise.
  • Can work effectively in both quiet and shared environments.

Key Requirements:

  • Must meet agency licensure requirements for fingerprinting.
  • Must have daily access to vehicle and valid drivers license.
