Job Description Peritoneal Dialysis or PD Travel RN (Registered Nurse)
  • Provide direct patient care and education for peritoneal dialysis patients in an outpatient (chronic) clinic setting.
  • Assess plan implement and evaluate individualized nursing care plans.
  • Train and support patients and caregivers on performing PD at home including equipment use aseptic technique and troubleshooting.
  • Monitor patient progress dialysis adequacy and treatment compliance through clinical assessments and lab results.
  • Collaborate with nephrologists dietitians social workers and other members of the care team to optimize patient outcomes.
  • Perform and document ongoing patient assessments including vital signs weight access site condition and fluid balance.
  • Respond to patient concerns and complications promptly escalating issues when necessary.
  • Educate patients and families on managing ESRD medications nutrition and infection prevention.
  • Maintain accurate and timely electronic health records in accordance with facility and regulatory standards.
  • Ensure compliance with all federal state and local regulations as well as organizational policies and procedures.
  • Participate in quality improvement initiatives and staff development activities.

Job Requirements
  • 1 year of recent experience as a home hemodialysis (HHD) RN
  • Graduate from an accredited nursing school
  • Current state specific nursing license in good standing or if applicable a compact state RN license in good standing
  • BLS certification required (Chronic and Acute settings)
  • ACLS certification required (Acute setting only)
