Job Description Peritoneal Dialysis or PD Travel RN (Registered Nurse)
Provide direct patient care and education for peritoneal dialysis patients in an outpatient (chronic) clinic setting.
Assess plan implement and evaluate individualized nursing care plans.
Train and support patients and caregivers on performing PD at home including equipment use aseptic technique and troubleshooting.
Monitor patient progress dialysis adequacy and treatment compliance through clinical assessments and lab results.
Collaborate with nephrologists dietitians social workers and other members of the care team to optimize patient outcomes.
Perform and document ongoing patient assessments including vital signs weight access site condition and fluid balance.
Respond to patient concerns and complications promptly escalating issues when necessary.
Educate patients and families on managing ESRD medications nutrition and infection prevention.
Maintain accurate and timely electronic health records in accordance with facility and regulatory standards.
Ensure compliance with all federal state and local regulations as well as organizational policies and procedures.
Participate in quality improvement initiatives and staff development activities.
Job Requirements
1 year of recent experience as a home hemodialysis (HHD) RN
Graduate from an accredited nursing school
Current state specific nursing license in good standing or if applicable a compact state RN license in good standing
BLS certification required (Chronic and Acute settings)
