Job Description Peritoneal Dialysis or PD Travel RN (Registered Nurse)
- Provide direct patient care and education for peritoneal dialysis patients in an outpatient (chronic) clinic setting.
- Assess plan implement and evaluate individualized nursing care plans.
- Train and support patients and caregivers on performing PD at home including equipment use aseptic technique and troubleshooting.
- Monitor patient progress dialysis adequacy and treatment compliance through clinical assessments and lab results.
- Collaborate with nephrologists dietitians social workers and other members of the care team to optimize patient outcomes.
- Perform and document ongoing patient assessments including vital signs weight access site condition and fluid balance.
- Respond to patient concerns and complications promptly escalating issues when necessary.
- Educate patients and families on managing ESRD medications nutrition and infection prevention.
- Maintain accurate and timely electronic health records in accordance with facility and regulatory standards.
- Ensure compliance with all federal state and local regulations as well as organizational policies and procedures.
- Participate in quality improvement initiatives and staff development activities.
Job Requirements
- 1 year of recent experience as a home hemodialysis (HHD) RN
- Graduate from an accredited nursing school
- Current state specific nursing license in good standing or if applicable a compact state RN license in good standing
- BLS certification required (Chronic and Acute settings)
- ACLS certification required (Acute setting only)
