Social Worker Older Persons Mental Health Service Burnie TAS

  • Full time contract up until 31st March
  • Community Setting
  • Join a collaborative multidisciplinary team


ACM Healthcare is working on behalf of Government Health Service in Tasmania providing Older persons community mental health care in the North West. Join a multidisciplinary team delivering person centred recovery orientated and trauma informed care in accordance with National Standards for Mental Health Services.

RESPONSIBILITIES COULD INCLUDE:

  • Information support and counselling for individuals and families.
  • Assessment of mental health problems.
  • Treatment advice and support in community and inpatient settings.
  • Home visits as appropriate or required.
  • Liaison and referrals with other health care professionals and service providers as needed.
  • Community education and health promotion.

MANDATORY QUALIFICATIONS AND EXPERIENCE:

  • Appropriate Social Work Degree or equivalent qualification and registration with relevant professional organisation
  • Working with Vulnerable People Check must be obtained prior to commencement
  • Ability to work until 31st March 2025
