RN-RPN - Chénchenstway - Long Term Care Facility

Are you a newly graduated RN or an experienced professional Providence Health Care is seeking dedicated Registered Nurses to join our 6long-term care facilities.

We are hiring for the following location:

• Chenchenstway at 7550 Rosewood St Burnaby BC

As a Staff Nurse youll be at the forefront of patient care working collaboratively within an interdisciplinary team to ensure patients/residents receive comprehensive support. Your responsibilities will include:

• Assess patients physical social spiritual and psychological needs
• Develop implement and evaluate individualized care plans
• Incorporate teaching into care plans to educate patients and families
• Make referrals to resources as needed
• Support fellow healthcare professionals
• Operate within the framework of Providence Health Cares mission vision and values

Education:

• Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives

Skills and Abilities:

• Demonstrated competence in performing CPR procedures.
• Experience in teamwork decision-making and conflict resolution
• Demonstrated a teaching aptitude
• Proficient in clinical leadership
• A strong desire to expand your professional skills and a commitment to lifelong learning in the nursing field

Your Day to Day

• Conduct thorough resident assessments through chart reviews observations interviews and collaboration with peers
• Develop implement and evaluate nursing care plans based on evidence-based practices prioritizing resident needs
• Adhere to established policies procedures and PHC standards while providing high-quality nursing care
• Monitor resident progress evaluate outcomes and adjust care plans as needed for optimal results
• Maintain accurate records of assessments care plans patient/resident status and discharge plans using manual and computerized systems
• Identify and address resident and family learning needs providing education and referrals as necessary
• Provide direction to staff promote teamwork and share clinical knowledge to enhance patient care
• Support nursing research initiatives and contribute to the improvement of resident care policies and procedures
• Advocate for residents rights and dignity facilitate discharge plans and collaborate with families and community resources
• Perform additional duties as assigned to support the healthcare team
