Pay :

HK$39485 to HK$87341 per month (HGPS Pt 16 to Pt 33) per month including Monthly Allowance

Up to 15% of total basic salary (after deducting the contribution of Mandatory Provident Fund by Hospital Authority) as contract gratuity may be offered to contract staff at the end of each 3 years during the 9-year contract period subject to satisfactory performance.

Key Responsibilities:

1. Provide psycho-social assessment and intervention.

2. Process discharge planning and post-discharge follow-up.

3. Co-ordinate and collaborate with different disciplines within and outside hospital to facilitate patient discharge and continuity of care.

4. Mobile community resources to enhance patients rehabilitation and reintegration into the community.

5. Support cluster Medical Social Service Provision.

6. Develop volunteer services and run groups/mass programs.

7. Conduct and participate in activities related to continuous quality improvement and service evaluation.

8. Perform any job-related duties as assigned by supervisor.

Entry Requirements:

1. (a) A degree in Social Work from a local university or equivalent.

OR

(b) A degree from a local university or equivalent and have successfully completed a one-year post-graduate course in Social Work at a local university or equivalent.

OR

(c) A degree from a local university or equivalent and have successfully completed a two-year post-graduate course in Social Work or possess a Masters degree in Social Work from a local university or equivalent.

2. Registration with and a valid registration certificate issued by the Hong Kong Social Workers Registration Board.

3. Fluency in written English and Chinese as well as spoken English and Cantonese.

Preferred Attributes / Exposure:

1. (a) Grade E in English Language (Syllabus B) and Chinese Language in the Hong Kong Certificate of Education Examination or equivalent.

OR

(b) Level 2 in English Language and Chinese Language in the Hong Kong Diploma of Secondary Education or equivalent.

2. Proficient in interpersonal and communication skills.

3. Strong drive to help (and utilize various social work approaches in patient care).

4. Good social work therapy skills.

5. Empathy.

6. Commitment to patient care.

7. Leadership.
