Job Summary
Position: Physical Therapist(PT) - Outpatient
Start Date: 9/15/2025
Duration: 13 weeks
Location: Fort Dodge IA 50501
Shift Type/Shift Times/Hours/Day: Days
Shift Notes: Mon - Fri: 8 hr shifts 40 hrs/week... Call: TBD... Weekends: TBD based on cases
Weekend Requirements: TBD based on cases
On-Call Requirements: TBD
COVID Vaccination: COVID-19 vaccinations are strongly recommended but not required
Travel Policy: Travelers must be COVID vaccinated; Medical/Religious Exemptions provided upon request

OT/Holiday Multiplier: 1.25x
NBO: 8 hrs
Patients per day: Acute: 10-12 & OP: 8-10
As a member of the interdisciplinary team contributes professional nursing knowledge and skills in the provision and management of care to patients through the application of the nursing process. We are looking for mostly outpatient but with one day per week in acute care.
Submission Requirements
- Min. of 1 year of PT experience
- OP experience required; however IP experience is encouraged due to working 1 day/week in Acute Care
- EPIC experience
- BLS
- Iowa
