Job Summary

Hospital Sisters Health System (HSHS) is seeking an RN Case Manager to join our collaboration with the patient/family social workers physicians interdisciplinary team and payer representative the Care Manager-RN ensures patient progression through the continuum of care in an efficient and cost effective manner. Identifies initiates and manages optimal care coordination and discharge planning to develop patient flow/throughput to enhance continuity of care smooth and safe transitions patient satisfaction patient safety and length of stay management. Provides support and expertise through comprehensive assessment planning implementation and overall evaluation of individual patient needs. Monitors the provision of cost effective care to ensure care is given in the most clinically appropriate setting. Facilitates compliance with regulatory requirements and organizational performance standards including demonstration of core competencies.

Education Qualifications
  • Associates Degree or Diploma in Nursing is required.
  • Bachelors Degree in Nursing is preferred.
Experience Qualifications
  • 2 years of clinical experience is required.
  • Prior case management experience is preferred.
  • Quality/Process Improvement experience is preferred.
Certifications Licenses and Registrations
  • License as a Registered Nurse (RN) in the state of practice is required. Wisconsin: RNs may practice with a valid multi-state RN license as outlined in the Nurse Licensure Compact rules.
  • Case Management certification (ACM CCM RN-BC) is preferred.
  • Basic Life Support Certification (BLS) is required within 60 days of hire.

Pay Range:

$34.21 - $51.31

A successful candidates actual pay rate will be based on several factors including relevant experience skills training certifications and education.
