About the Home Health/Hospice Team
Our Home Health and Hospice teams pride themselves on providing top quality care in the home to those individuals requiring additional healthcare services or during their end-of-life journey.
Mission for Position
Provide skilled nursing care and general health guidance to patients in the home health and hospice setting so every patient and family receives personalized compassionate care.
Duties & Responsibilities
- Correctly implement the clientPlan of Care established by the physician
- Observe proper infection control and safety procedures
- Supervise health assistant services
- Coordinate care team to meet patient and family needs during end of life
Position Type
- Casual Status. One holiday per year and call rotation one evening every other week and approximately every 11thweekend are required.
Qualifications
Qualifications
- Current MN RN licensure
- Current valid MN Drivers License and current auto insurance
Ideal Candidate
- Thrives working independently but knows when to call on their team
- Has excellent communication skills
- Can make decisions with limited information
- Manages patient-family dynamics with ease
- Is eager to learn and grow
- Hospice experience preferred but not required
