Summary: Mercy Regional Medical Center is looking for a locum Urologist to help fill the dates detailed below. Candidates must be available for any of the stints below to be considered for ongoing dates. Site anticipates the need of 3 weeks per month for all locums coverage ongoing.

RATE CAPS FOR SUBMISSION:Malpractice Must be included in rates.
Orientation $250 - half the hourly/callback rate kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
Hourly $500 - Per hour not OT
Holiday $750 - Must be 1.5x the hourly rate. Per holiday hour no OT
Nightly Call $1000 - Per night call to hold the pager to include 2.5 hours gratis
Holiday Nightly Call - $1500 - Per night call to hold the pager to include 2.5 hours gratis
On Call (Daily) $3500 - Per 24 hours to include 4 hours gratis
24 Hour Holiday Call Rate - $5250 - Per 24 hours to include 4 hours gratis
Call Back $500 Per hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.
Holiday Call Back $750 Per holiday hour of patient contact hours after gratis hours are completed gratis prorate to MSA terms.
Average Call Back: Night Call 1.7 hours; 24 hour call 3.2 hours
Dates of Coverage: - All shifts end the following day at 7a.

- Ongoing dates will be needed and will begin starting Nov 3 2025 as schedules are released.
Details:
- Site needs 3 weeks per month for locum coverage in total.
- Monday-Friday - 7a-5p 5p-7a (night call) & Saturday-Sunday - 24hr call (7a-7a)
- Setting: Inpatient Outpatient Emergency Room Clinic Call Trauma Level III
- FTE: .5
- Required Procedures/Job Duties: Seeing Clinic patients & general urology surgeries
- OR time for various procedures
- EMR: EPIC
- Support Staff: MA NP/PA RN
- Reason for Coverage: 3 locums require 6 month break.
EMR Required Training:
- Module (learn lessons) Must be completed prior to virtual EMR training to attend the training.
- EMR Onsite training Only able to attend if Modules are completed before training. Onsite EMR training will be schedule the morning of providers first day.
- All candidates must complete the following EMR questionnaire and detail it in the notes section of the candidates submission at time of presentation.

- When was the last time (MM/YYYY) that they used Epic
- What is the name of the last facility the provider used EPIC

Travel Preferences:
- Preferred Lodging: Must follow CSH travel policy.
- Air Travel: Okay.
- Please detail any anticipated travel exceptions for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.
Credentialing Information:
- Timeframe: 60-90 days

Requirement description :
- Board Certified Urology - Required will NOT accept BE candidates
- Active CO License - Required or IMLC (LOQ in hand at time of presentation)
- Active BLS - Required for credentialing
- Active DEA - Required for credentialing (site will consider a transferred DEA however must keep this DEA for the duration of the assignment not just for shifts worked.)
- Candidates must detail which of the stints open they are able to cover through Dec 2025 and must be available for any stint in job description to be considered for ongoing dates - Required
- Must list # of shifts per month ongoing the provider is able to offer to the team - Required
- Must acknowledge candidate understands the EMR training requirements set forth in job description and will complete in this order to avoid missing their start date - Required
- Clean Malpractice - Required willing to accept 1 case up to $300k - Candidates who present with a self inquiry NPDB report at time of presentation will be considered with priority over those that do not.
