Job Description:

As an After Hours / On Call RN youll be the calm in the storm providing critical support when patients and families need it most. Provide urgent compassionate nursing care after hours in alignment with Elara Carings mission
Assess patient and family needsphysical emotional psychosocial and spiritual
Communicate significant changes or concerns to the Clinical Supervisor physician or facility staff
Educate patients families and facility staff on hospice philosophy pain control and symptom management
Offer support during end-of-life transitions and perform bereavement assessments
Collaborate with the Interdisciplinary Team to ensure the best possible quality of life for each patient
Graduate of an accredited nursing program (Diploma Associate or Bachelors)
Current RN license in the state of practice
At least 1 year of clinical nursing experience (hospice or home health preferred)
Willingness to travel up to 50%
Physical ability to lift 50100 lbs.
Valid drivers license insurance and reliable transportation
