Under the guidance of the Resident Care Manager and in close collaboration with a diverse healthcare team the Licensed Practical Nurse (LPN) delivers top-notch nursing care to long-term care residents in addition to providing emotional comfort and support to residents and their families. LPNs in long-term care have the unique opportunity to build deep and lasting relationships with residents/families/caregivers collaborate with a compassionate and knowledgeable interdisciplinary care team and contribute to an overall sense of home for residents. What You Bring
Education:
- Graduation from a recognized program for Practical Nurses or an equivalent combination of education training and experience
- Registration with the British Columbia College of Nurses and Midwives as a practicing LPN registrant
- Current basic cardiac life support certification
Skills & Abilities:
- Demonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service
- Physical ability to carry out the duties of the position
- Ability to apply decision making and analytical skills
- Ability to role model conflict resolution strategies
- Ability to prioritize and organize work
- Ability to teach and identify patient/resident learning needs

Your Day to Day
- Provide safe and respectful nursing care adhering to established policies individualized care plans and priorities
- Conduct thorough assessments monitor signs/symptoms and communicate with the RN/RPN as needed
- Prepare patients for procedures transfers and admissions coordinating with the interdisciplinary team
- Administer medications per protocol monitor effects and maintain communication with the team
- Utilize evidence-based resources to stay updated on nursing practices
- Respond to incidents such as codes injuries spills and alarms following procedures
- Assist with staff orientation demonstrating procedures and providing support
- Address patient/resident and family learning needs provide educational materials and make referrals
- Attend care planning meetings provide input and advocate for patients/residents
- Collaborate with the team to develop and evaluate individualized care plans
- Assist with discharge planning based on patient status and external supports
- Operate and clean equipment report malfunctions and handle hazardous materials. Perform clerical duties such as answering phones faxing and maintaining records
