Description

Summary of Position

Provides intensive case management services to individuals and families experiencing homelessness assisting them in quickly securing stable housing through a time-limited rental assistance program while also connecting them to necessary supportive services to maintain housing stability and promote self-sufficiency.

Examples of Duties and Responsibilities

  1. Client Intake and Assessment:
    1. Conduct comprehensive assessments of individuals and families experiencing homelessness to identify housing barriers needs and eligibility for the Rapid Rehousing program.
    2. Develop individualized housing plans with clients including goals timelines and necessary support services.
  2. Housing Search and Placement:
    1. Assist clients in actively searching for suitable housing options within the private rental market.
    2. Negotiate with landlords on behalf of clients to secure housing placements.
    3. Provide rental assistance and security deposit assistance as per program guidelines.
  3. Case Management:
    1. Regularly meet with clients in their home to provide inspections and safety checks to ensure that housing standards are being upkept and to monitor progress towards housing stability and address any challenges.
    2. Provide ongoing support services including budgeting employment assistance benefits navigation mental health referrals and substance abuse treatment referrals.
    3. Advocate for clients with landlords property management companies and other service providers.
  4. Documentation and Reporting:
    1. Maintain detailed case files with accurate documentation of client interactions housing searches service referrals and progress updates.
    2. Prepare regular reports on client outcomes and program performance according to agency standards.
  5. Community Collaboration:
    1. Build and maintain relationships with local landlords housing providers social service agencies and other community partners to access housing options and support services for clients
    2. Participate in outreach efforts to identify individuals and families in need of Rapid Rehousing services.
    3. Participate in Continuum of Care (COC) and other monthly meetings and trainings as needed.
  6. Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above

Minimum Qualifications

A bachelors degree in Social Work Human Services or another related field preferred. Zero to three (0-3) years of related experience required. Knowledge of housing market dynamics homeless service systems and relevant government regulations preferred. Strong case management skills including assessment goal setting problem-solving and crisis intervention preferred. Excellent communication and interpersonal skills to build rapport with clients and collaborate with community partners. Proficiency in data entry and case management software. A valid drivers license is required.

Physical Requirements

Sit for extended periods of time. The employee is required to walk or move up to 200 feet from primary desk. The employee is occasionally required to stand and stoop kneel or crouch. The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision and distance vision.
