Overall Responsibility
Serves as a medication therapy resource to patients staff and physicians. Assesses new and ongoing drug therapy for appropriateness safety and ability to produce optimal outcomes for each individual patient. Assists in formulary management by implementing approved therapeutic substitutions dosage adjustments and optimal timing of medication. Recommends appropriate therapeutic alternatives and monitors patient outcomes. Processes medication orders in a timely efficient and accurate manner. Leads and checks work of Pharmacy Technician as directed by or in the absence of Pharmacy Manager.
Key Tasks and Responsibilities
Essential Functions
- Prioritizes medication orders.
- Carefully checks each medication order utilizing patient history (where appropriate) age weight height laboratory data and allergies to ensure that it is optimal and individualized for the patient.
- Reviews patient profiles (for drug interactions allergies therapeutic duplication appropriate medication route etc.) prior to entering new medication orders.
- Processes medication orders in a timely efficient and accurate manner.
- Provides appropriate and accurate information regarding drug therapy to physicians nurses patients and employees.
- Reports Adverse Medication Events.
- Suggests conversion to oral dosage route when appropriate.
- Suggests appropriate formulary/therapeutic options to non-formulary medications.
- Carefully checks all work performed by technicians.
- Leads the work of technicians as directed by or in the absence of the Pharmacy Manager.
- Maintains accurate controlled medication records including following proper procedures and record keeping for the receipt dispensing return and destruction of controlled medications.
- Collaborates with nursing staff to ensure floor stock levels and product are appropriate and that the overall pharmaceutical needs of the unit are met.
- Prepares special and complex medications accurately (e.g. IV admixtures and extemporaneous dosage forms).
- Follows all departmental policies and completes assigned duties within specified time frames.
- Understands and follows USP <797> guidelines to prepare and verify compounded sterile products.
Non-Essential Functions
- Performs other duties as assigned.

- Maintains patient and hospital confidentiality at all times.
- Informs Manager of applicable day-to-day situations.
Licensure and Certification
- Current Pharmacist licensure in good standing within the state of Kansas.
Experience Required
- Bachelors degree in Pharmacy required.
- Doctorate degree in Pharmacy preferred.
- Minimum of one-year pharmacist experience in an acute care facility preferred.
