Compensation is in accordance withSCHADS Award Level 5.
- Develop and implement strengths-based solution-focused case plans in partnership with clients
- Support transitions to long-term housing through warm referrals and coordinated outreach
- Advocate across housing health legal and community systems to meet client needs
- Collaborate with internal teams and external partners to connect clients with relevant services
- Maintain accurate records and contribute to service development and continuous improvement
- A degree qualification in Social Work Psychology or a related field
- Demonstrated experience in case management within a homelessness or community services setting
- Knowledge of homelessness trauma and housing systems (including the Victorian Housing Register)
- Strong communication and advocacy skills
- A collaborative spirit and ability to build positive relationships with clients and stakeholders
- Cultural awareness and a commitment to inclusive practice
- A deep alignment with The Salvation Armys mission values and commitment to justice
- Current Australian Drivers Licence
