Job Description
Community Health Center of the North Country seeks a compassionate and dedicated Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) to join our Federally Qualified Health Centers Behavioral Health this unique opportunity youll provide individual therapy and counseling services in an outpatient primary care setting that embraces a fully integrated model of care.

Reports to: Medical Director / Practice Manager
Rate of Pay: $60000 - $80000 for full-time status; part-time or per diem shall be prorated based upon scheduled hours
Work Schedule: Flexible within Monday - Friday daytime office hours; no evenings weekends or major holidays; no on-call; full-time part-time or per diem schedules available
The selected candidate may be eligible for student loan repayment through the National Health Service Corps (NHSC) as we are an NHSC-approved site. Program eligibility details may be found HERE.
Position Summary: Responsible for providing individual therapy and/or counseling services to Health Center patients. This position also provides input and support for staff involved in the care of the patients.
Primary Responsibilities:
- Perform psycho/social assessment.
- Develop a therapeutic counseling regimen to meet the needs of the individual based upon assessment and input from caregivers/family.
- Coordinate treatment plan with the psychiatrist referring individuals as necessary for psychiatric consult and prescription.
- Provide ongoing counseling as indicated and follow up if a psychiatric consultation is made.
- Maintain patient records per agency policies and procedures.
- Provide ongoing supervision of MSWs as necessary and appropriate.
- Perform other duties as requested by the supervisor.
- Masters Degree in Social Work required.
- Current registered NYS LMSW license required.
- At least 3 years of post-M.S.W. supervised experience in diagnosis psychotherapy and assessment-based treatment planning with a current registered NYS LCSW license preferred
- Experience with electronic medical records is preferred.
- Strong organizational and planning skills.
- Ability to maintain good working relationships with others.
- Exhibits a high level of professionalism and confidentiality in dealing with issues.
- Sedentary work; ability to occasionally lift and carry up to 25 lbs.
- Ability to sit for long periods.
- Ability to tolerate exposure to a computer monitor/screen.
- Ability to tolerate the extended use of a computer keyboard.
