About us
Wellington Aboriginal Corporation Health Service (WACHS) is an Aboriginal community-controlled health organisation that aims to empower Aboriginal people to take control of their individual family and community health and wellbeing needs through our clinical and community programs.
Our main services are in Wellington Dubbo Western Sydney Penrith Nepean and Blue Mountains and we provide outreach services to other towns and communities through our regional programs.
About the role
This role provides direct patient contact and clinical support in the areas of Maternal and Child Health and Chronic Disease Management. The role requires participation as a team member in health promotional activities in all areas of the Healthy for Life program and the Wellington Clinic.
Key Responsibilities
Liaise between clients and clinic staff and assist with the follow up of clients treatments
Provide appropriate referral services referring clients to appropriate services and community health
Monitor local Aboriginal Health problems and needs and advise the Clinic Manager and/or professional staff
Participate in or carry out preventive and health education activities under the supervision of GPs and or Nursing Staff
Promote healthy lifestyle in the community
Collect collate and coordinate health statistics and information processes for the advancement of Aboriginal health and well-being
Implement and evaluate public health strategies programs and services directed to the needs of the Aboriginal community of the region
Undertake primary care tasks within the boundaries of your qualifications
Work closely with the GPs and relevant clinic staff to assist with all aspects of the development and delivery of health care plans
Develop and maintain contact with communities regarding Indigenous health
Assist in the development and delivery of health education programs
Provide reports as required on time
Liaise with allied health professionals and specialist as required
Liaise with other AMSs and hospitals for client medical and medication records as necessary
Ensure good rapport and communication with external health services
Actively encourage and promote continuous improvement
Liaise with clients for screenings and tests as required
Demonstrated knowledge and understanding of respect for and compliance with culturally appropriate ways of working with Aboriginal individuals families and communities
Essential Criteria
Aboriginality
Certificate III Certificate III in Aboriginal and/or Torres Strait Islander Primary Health Care or Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care Practice or working towards achieving the qualification
Demonstrated experience of working independently and/or showing initiative in relation to patient care
Understanding and knowledge of specialist medical allied health and support services locally
Excellent communication time management and organizational skills.
Knowledge and understanding of the issues that impact on Aboriginal people and Aboriginal communities
Demonstrated knowledge of the primary health care setting in a variety of locations or ability to develop this knowledge base in the brief period of time.
Computer literacy and a high standard of report writing
