Program: HERO (Housing Empowering Recovery Opportunities)
Position: Program Manager 1
Location: New Haven
Salary: $/year
Schedule: Full time 40 hours 1st shift Monday-Friday
Program Summary:
The Housing Empowering Recovery Opportunities (HERO) program is a permanent supportive housing program that works with homeless individuals with substance use diagnoses by assisting them in moving into permanent housing and providing intensive case management services. Services include providing in-home individualized case management coordination of referrals and appointments development of tenancy-based skills landlord mediation apartment searching and life skill building activities. The programs goal is to help people maintain their subsidized housing and improve their day-to-day functioning.
Position Summary:
Responsible for assisting the Program Director with program operations and quality assurance. Direct supervision and Coordination and ongoing training with CAMIS/HMIS/core DMHAS training for program case managers providing weekly monthly or quarterly reports according to agency and funder requirements. Provide direct supervision and leadership to the case managers. Monitor program census admit new clients and assign cases. Closely collaborate with the regional Coordinated Access Network (CAN) around program referrals. Organize and facilitate case conferences with outside agencies as needed. Assist Program Director with the coordinating administrative meetings with funders for ongoing program review and evaluation of reports. The Program Manager reports to the Program Director.
Working within our Housing and Homeless Services the Case Manager 2 works with the HERO program staff to provide services to stabilize and reunite families experiencing complex issues such as substance use behavioral health matters and homelessness.
Requirements:
- Bachelors Degree in related field
- 3 years of related leadership experience
- Valid Connecticut Drivers License
- Regular and predictable attendance is required
Orientation:
If selected for employment with The Connection newly hired employees will be required to attend a two (2) day mandatory New Hire Orientation at our Roscommon office located in Middletown.
