Job Description

Home Health Intake Specialist

Are you passionate about helping others and making a difference in the lives of patients and families Join our dedicated team as an Intake Specialist and play a vital role in connecting patients to the care they need in the comfort of their homes.


Full Time- Starting at $18 an hour


The Intake Specialist plays a critical role in the onboarding of new patients into home health

services. This position ensures timely accurate and compassionate coordination of referrals patient

information and communication between referral sources patients clinicians and administrative staff.

Key Responsibilities:

Referral Management:

  • Receive and process incoming referrals from hospitals physicians and other healthcare providers.
  • Verify insurance eligibility and benefits.
  • Ensure all required documentation is complete and compliant with regulatory standards.

Patient Coordination:

  • Contact patients and/or caregivers to confirm services and gather necessary information.
  • Schedule initial assessments and coordinate with clinical staff.
  • Maintain accurate and up-to-date patient records in the EMR system.

Communication & Collaboration:

  • Serve as a liaison between referral sources patients and internal departments.
  • Communicate effectively with clinical and administrative teams to ensure smooth transitions of care.
  • Provide updates to referral sources as needed.

Compliance & Documentation:

  • Ensure all intake documentation meets Medicare and other payer requirements.
  • Maintain confidentiality and comply with HIPAA regulations.
  • Assist with audits and quality assurance initiatives as needed.

Qualifications:

  • High school diploma or equivalent required; associate or bachelors degree preferred.
  • Minimum of 2 years of experience in healthcare intake preferably in home health or hospice.
  • Knowledge of medical terminology insurance verification and EMR systems.
  • Strong organizational skills and attention to detail.
  • Excellent communication and interpersonal skills.
  • Ability to multitask in a fast-paced environment.

Preferred Skills:

  • Experience with home health EMR platforms
  • Familiarity with Medicare and regulations.
